摘要
目的比较跟骨牵引与石膏外固定在Denis-Weber C型踝关节骨折术前治疗中应用的效果。方法回顾性分析自2017-01—2018-12行切开复位内固定治疗的100例Denis-Weber C型踝关节骨折,52例入院当天进行跟骨牵引治疗(跟骨牵引组),48例进行石膏外固定(石膏固定组)。比较2组术前张力性水泡发生率、术前皮肤坏死发生率、术前消肿时间、住院时间,以及入院后1、3、5 d患肢肿胀值。结果跟骨牵引组入院后1、3、5 d患肢肿胀值小于石膏固定组,跟骨牵引组术前消肿时间、住院时间较石膏固定组明显缩短,跟骨牵引组术前张力性水泡发生率与皮肤坏死发生率较石膏固定组明显降低,差异有统计学意义(P<0.05)。结论Denis-Weber C型踝关节骨折患者入院时进行跟骨牵引治疗可加快骨折周围软组织肿胀消退,缩短术前等待时间,并在一定程度上降低术中骨折复位难度。
Objective To compare the effect of calcaneal traction and plaster external fixation in the preoperative treatment of Denis-Weber C ankle fractures.Methods A retrospective analysis was performed on 100 Denis-Weber C ankle fractures treated with open reduction and internal fixation from January 2017 to December 2018.Fifty-two patients received calcaneal traction(calcaneal traction group)on the day of admission,and 48 patients received plaster external fixation(plaster fixation group).The incidence of tension blisters,preoperative skin necrosis,preoperative detumescence time,hospital stay,and limb swelling values on day 1,3,and 5 after admission were compared between the two groups.Results In the calcaneal traction group,the swelling value of the affected limb on day 1,3,and 5 after admission was lower than that of the plaster fixation group;the preoperative detumescence time and hospital stay of the calcaneal traction group were significantly shorter than those of the plaster fixation group;the incidence of tension blisters and skin necrosis in the calcaneal traction group was significantly lower than that in the plaster fixation group,and the difference was statistically significant(P<0.05).Conclusion In patients with Denis-Weber C ankle fractures,calcaneal traction on admission can accelerate the dissolution of soft tissue swelling around the fracture,shorten the waiting time before surgery,and reduce the difficulty of intraoperative fracture reduction to a certain extent.
