地特胰岛素注射液联合短效胰岛素对儿童糖尿病的疗效

The Effect of Insulin Detemir Injection Combined with Short-acting Insulin on Childhood Diabetes and Its Influence on Auxiliary Th1 Cytokines

摘要 目的观察地特胰岛素注射液在儿童糖尿病患者中的疗效。方法选取2014年3月~2020年3月我院收治的儿童糖尿病患者72例,按照随机数字表法分为研究组和对照组,各36例。对照组给予常规药物治疗,研究组给予地特胰岛素注射液联合短效胰岛素治疗,比较两组临床疗效、血糖以及BMI指标变化、血清Th1(IL-2、TNF-α)及Th2(IL-4、IL-10)细胞因子水平变化及血糖相关不良事件发生率。结果研究组治疗总有效率为94.44%,高于对照组的69.44%(P<0.05);两组FBG、2h PG、HbAlc以及BMI较治疗前改善,且研究组改善水平优于对照组,差异有统计学意义(P<0.05);两组IL-2、IL-4、IL-10、TNF-α水平较治疗前改善,且研究组的改善水平优于对照组(P<0.05);研究组血糖相关不良事件发生率为27.78%,低于对照组的61.11%(P<0.05)。结论地特胰岛素注射液联合短效胰岛素治疗儿童糖尿病疗效确切,可对辅助性Th细胞因子进行有效抑制,促进患者血糖水平改善,且血糖相关不良事件发生率低。 Objective To observe the efficacy of insulin detemir injection in children with diabetes and its influence on auxiliary Th1 cytokines.Methods A total of 72 pediatric diabetic patients admitted to our hospital from March 2014 to March 2020 were selected and divided into a study group and a control group according to a random number table,with 36 cases in each group.The control group was given conventional medication,and the study group was given insulin detemir injection combined with short-acting insulin therapy.The clinical efficacy,blood glucose and BMI index changes,serum Th1 and Th2 cytokine levels,and blood glucose-related adverse events were compared between the two groups.Results The total effective rate of treatment in the study group was 94.44%,which was higher than 69.44% in the control group(P<0.05);the two groups of FBG,2h PG,HbAlc and BMI were improved compared to before treatment,and the improvement level of the study group was better in the control group(P<0.05);the serum Th1 and Th2 cytokine levels of the two groups were improved compared with before treatment,and the improvement of the study group was better than the control group(P<0.05);The incidence of blood glucose-related adverse events in the group was 27.78%,which was lower than 61.11% in the control group(P<0.05).Conclusion Insulin detemir injection combined with short-acting insulin has a definite effect in the treatment of childhood diabetes.It can effectively inhibit auxiliary Th1 cytokines,promote the improvement of blood glucose levels in patients,and has a low incidence of blood glucose-related adverse events.
