摘要
目的探究单纯性上尿路感染(UUTI)患者病原菌分布及血清单核细胞趋化蛋白-1(MCP-1)、脂联素(ADPN)、白细胞介素-10(IL-10)的变化情况。方法选取2018年1月-2019年6月三门县人民医院就诊的单纯性上尿路感染患者200例为感染组;选取同期来医院体检的健康人群200名为对照组;收集患者一般临床资料进行比较分析;采用全自动微生物分析系统鉴定菌株,采用酶联免疫吸附法检测两组血清MCP-1、ADPN及IL-10的水平。绘制受试者工作特征曲线(ROC),初步判定三种指标以及联合指标对单纯性上尿路感染的诊断价值。结果病原菌以大肠埃希菌(47.75%)为主;与对照组比,感染组血清MCP-1、IL-10水平均升高(P<0.01),血清ADPN水平下降(P<0.01)。ROC曲线分析,MCP-1、ADPN、IL-10以及MCP-1联合IL-10四种指标的ROC曲线下面积分别为0.915、0.894、0.795以及0.939,在各指标取最佳临界值时灵敏度分别为79.54%、86.52%、78.01%和89.59%,特异度分别为90.54%、75.08%、71.35%和88.04%。结论大肠埃希菌是导致单纯性上尿路感染的主要致病菌;血清MCP-1、IL-10水平在UUTI患者中上升,ADPN水平下降;检测患者血清MCP-1、ADPN及IL-10水平对单纯性上尿路感染的临床诊断与治疗有着一定的临床意义且联合检测的应用价值更高。
OBJECTIVE To explore the distribution of pathogens isolated from patients with simple upper urinary tract infection(UUTI)and the changes of serum monocyte chemotactic protein-1(MCP-1),adiponectin(ADPN)and interleukin-10(IL-10).METHODS Totally 200 patients with simple UUTI who were treated in People’s Hospital of Sanmen County from Jan 2018 to Jun 2019 were assigned as the infection group,meanwhile,200 healthy people who received physical examination were set as the control group.The baseline clinical data were collected from the patients,the strains were identified by using automatic microbial analysis system,the levels of serum MCP-1,ADPN and IL-1 of the two groups were detected by means of enzyme-linked immunosorbent assay.The receiver operating characteristic(ROC)curve was drawn to preliminarily determine the values of the three indexes and their combination in diagnosis of simple UUTI.RESULTS Escherichia coli was the predominant species of pathogen,accounting for 47.75%.The levels of serum MCP-1 and IL-10 of the infection group were significantly higher than those of the control group(P<0.01),the serum ADPN level of the infection group was significantly lower than that of the control group(P<0.01).ROC curve analysis showed that the areas under ROC curve of MCP-1,ADPN,IL-10 and MCP-1 combined with IL-10 were 0.915,0.894,0.795 and 0.939,respectively;when the optimal thresholds of the indexes were taken,the sensitivities were 79.54%,86.52%,78.01%and 89.59%,respectively;the specificities were 90.54%,75.08%,71.35%and 88.04%,respectively.CONCLUSION E.coli is the predominant species of pathogen causing the simple UUTI.The levels of serum MCP-1 and IL-10 of the UUTI patients are elevated,the ADPN level is reduced.The serum MCP-1,ADPN and IL-10 show certain clinical significance in the clinical diagnosis and treatment of the simple UUTI,and the joint detection of the indexes has higher application value.
