NF-κB阳性非小细胞肺癌合并感染患者病原菌分布及耐药性分析 被引量:4

摘要 目的探究核因子-κB(NF-κB)阳性非小细胞肺癌合并感染患者的病原菌分布和耐药情况。方法回顾性分析2015年1月-2018年12月在郑州大学附属郑州中心医院及南京医科大学附属老年医院收治非小细胞肺癌合并感染的非外科手术治疗患者,根据NF-κB表达情况,分为NF-κB阳性组44例,NF-κB阴性组44例。对两组患者病原菌分布、耐药菌株数量进行分析。结果 88例非小细胞肺癌合并感染患者的样本中共分离出病原菌159株,其中革兰阳性菌52株(32.70%),革兰阴性菌95株(59.75%);对耐药性研究发现,在革兰阳性菌对抗菌药物中的耐药率全部>50%的有青霉素和四环素以及阿莫西林、氯唑西林;并且克林霉素也有着较高的耐药率。对革兰阴性菌具有较高的耐药率的有头孢曲松、头孢唑林和磺胺甲噁唑/甲氧苄啶。NF-κB阳性组与NF-κB阴性组两组患者病原菌构成及部分耐药菌株数量的差异,存在统计学意义(P<0.05)。结论非小细胞肺癌合并感染患者,耐药情况不容乐观,可能与NF-κB通路存在联系,值得深入研究。 OBJECTIVE To explore the distribution and drug resistance of pathogens isolated from nuclear factor-κB(NF-κB)positive non-small cell lung cancer patients complicated with infection.METHODS The non-small cell lung cancer patients complicated with infection who received non-surgery therapies in Zhengzhou Central Hospital Affiliated to Zhengzhou University and Geriatric Hospital Affiliated To Nanjing Medical University from Jan 2015 to Dec 2018 were retrospectively analyzed,the enrolled patients were divided into the NF-κB-positive group with 44 cases and the NF-κB-negative group with 44 cases according to the status of expression of NF-κB.The distribution of pathogens and number of drug resistant strains were observed.RESULTS A total of 159 strains of pathogens were isolated from the specimens of 88 non-small cell lung cancer patients complicated with infection,52(32.70%)of which were gram-positive bacteria,95(59.75%)were gram-negative bacteria.The study of drug resistance showed that the drug resistance rates o f the gram-positive bacteria to penicillin,tetracycline,amoxicillin and cloxacillin were more than 50%,and the drug resistance rate to clindamycin was also high;the gram-negative bacteria were highly resistant to ceftriaxone,cefazolin and sulfamethoxazole-trimethoprim.There were significant differences in the constituent ratios of pathogens and the number of strains of drug-resistant isolates between the NF-κB-positive group and the NF-κB-negative group(P<0.05).CONCLUSION The drug resistance of the non-small cell lung cancer patients complicated with infection is not optimistic,which may be related to the NF-κB pathway,it is worthy to be further promoted.
