摘要
目的探讨基于扩散峰度成像(DKI)脑小血管病(CSVD)抑郁患者杏仁核微结构的变化。方法选取CSVD患者64例,分为抑郁组36例和对照组28例,采用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评估。经DKI扫描提取杏仁核参数,包括峰度参数:轴向峰度(Kax)、峰度各向异性分数(KFA)、平均峰度(Kmean)、径向峰度(Krad);扩散参数:平均扩散系数(Dmean)、径向扩散系数(Drad)及轴向扩散系数(Dax)。结果抑郁组HAMD评分、HAMA评分、左侧杏仁核Dax、Dmean、Drad明显高于对照组[16.0(12.0,22.0)分vs 0(0.0,4.0)分,12.0(6.0,21.0)分vs 0(0.0,3.0)分,(1.76±0.28)μm^2/ms vs(1.55±0.24)μm^2/ms,(1.51±0.25)μm^2/ms vs(1.35±0.21)μm^2/ms,(1.42±0.23)μm^2/ms vs(1.24±0.20)μm^2/ms,P<0.01],左侧杏仁核KFA明显低于对照组[0.28(0.24,0.33)vs 0.32(0.29,0.35,P=0.006]。左侧杏仁核Dax、Drad与HAMD评分呈正相关(P<0.05);右侧杏仁核Drad与HAMD评分呈正相关(P<0.05),KFA与HAMD评分呈负相关(P<0.05)。结论DKI可发现CSVD抑郁患者杏仁核微结构的变化,CSVD性抑郁患者左侧杏仁核微结构的改变可能早于右侧。
Objective To study the changes of amygdala microstructure detected in patients with depression induced by CSVD based on DKI.Methods Sixty-four CSVD patients were divided into depression group(n=36)and control group(n=28).Their neuropsychology was assessed according to their HAMD scale and HAMA scale score.Following kurtosis parameters were extracted based on DKI,including abundance parameters:Kax,KFA,Kmean,Krad,and diffusion parameters:Dmean,Drad,Dax.Results The HAMD scale score,HAMA scale score,Dax,Dmean and Drad of the left amygdala were significantly higher while the KFA was significantly lower in depression group than in control group[16.0(12.0,22.0)vs 0(0.0,4.0),12.0(6.0,21.0)vs 0(0.0,3.0),1.76±0.28μm^2/ms vs 1.55±0.24μm^2/ms,1.51±0.25μm^2/ms vs 1.35±0.21μm^2/ms,1.42±0.23μm^2/ms vs 1.24±0.20μm^2/ms,P<0.01;0.28(0.24,0.33)vs 0.32(0.29,0.35,P=0.006].The Dax and Drad were positively related with the HAMD scale score of left amygdala(P<0.05),the Drad was positively related with the HAMD scale score of right amygdala(P<0.05)and the KFA was negatively related with the HAMD scale score of right amygdala(P<0.05).Conclusion DKI can show the changes of amygdala microstructure in patients with depression induced by CSVD.Changes of left amygdala microstructure occur earlier than those of right amygdala microstructure in the progression of depression induced by CSVD.
