血液净化治疗有机磷农药中毒患者的应用研究 被引量:1

摘要 目的研究血液净化(包括血液滤过、血液灌流、血浆置换)在治疗有机磷中毒患者过程中的作用。方法回顾性选取我院重症监护病房(ICU)收治的18例有机磷中毒的患者,根据治疗方法不同将患者分为对照组和治疗组,均予急诊清洗皮肤、洗胃、灌肠导泻、阿托品及胆碱酯酶复能剂应用,治疗组在上述治疗基础上加用血液净化治疗,对比两组患者的意识恢复时间、治疗1周之后全血胆碱酯酶活性、呼吸衰竭和肾衰竭发生率、住院时间。结果治疗组患者的意识恢复时间、住院时间均明显短于对照组(P<0.05),差异有统计学意义;治疗组患者治疗1周之后全血胆碱酯酶活性明显高于对照组(P<0.05),差异有统计学意义;治疗组的呼吸衰竭和肾衰竭发生率明显低于对照组(P<0.05),差异有统计学意义。结论血液净化治疗可有效缓解有机磷中毒患者的中毒症状,临床效果显著,可积极应用。 Objective To study the effect of blood purification(including hemofiltration,blood perfusion,plasma exchange)in the treatment of patients with organophosphate pesticide poisoning.Methods Eighteen patients with organophosphorus poisoning admitted to the intensive care unit(ICU)of our hospital were retrospectively divided into control group and treatment group according to the different treatment methods.All patients were treated with emergency skin cleaning,pump stomach,clysis atropine and cholinesterase regenerative application,the treatment group patients were treated by blood purification treatment based on the above treatment,the recovery time of consciousness,blood cholinesterase activity after one week of treatment incidence,respiratory failure and renal failure,hospital time were compared between the two groups.Results The recovery time of consciousness and hospital time of the treatment group were significantly shorter than those of the control group(P<0.05).The difference was statistically significant.The cholinesterase activity of the treatment group was significantly higher than that of the control group after one week of treatment.(P<0.05),the difference was statistically significant.The incidence of respiratory failure and renal failure in the treatment group were significantly lower than that in the control group(P<0.05),and the difference was statistically significant.Conclusion Blood purification treatment can effectively alleviate the symptoms of poisoning in patients with organophosphate poisoning.The clinical effect is significant and thus suited for clinical application.
