摘要
高位脊髓损伤患者病情危重、治疗费用高、预后差。在当前医疗条件下,医务人员遇到的一些伦理问题如果处理不当,对诊疗工作的开展、患者的救治乃至医患关系都会造成不良影响。基于此,围绕生命神圣论与生命质量论、价值论的冲突,患方选择与患者自身利益的冲突,对患者告知病情与维护患者积极心理的冲突等伦理问题展开分析和论证,提出应遵循的伦理原则有:注重生命价值、提高生命质量原则,权衡患方意见与患者自身利益原则,心理支持原则;并提出医学伦理视角下"制度建设"的建议,为相关专业医务人员的临床实践和我国医疗卫生相关法律法规的制定提供一些参考。
Patients with high-level spinal cord injury have critical condition,high treatment cost and poor prognosis.Under today’s medical conditions,if some ethical problems encountered by medical staff are not handled properly,it will adversely affect the implement of diagnosis and treatment,the treatment of patients and even the doctor-patient relationship.Based on this,the ethical issues such as the conflict between the sacredness of life theory,the quality of life theory and the value theory,the conflict between the patient’s choice and the patient’s own interests,the conflict between deciphering patients and maintaining the patients’positive psychology were analyzed and demonstrated,the ethical principles to be followed are proposed:the principle of paying attention to the value of life and improving the quality of life,principle of weighing the patient’s opinion against the patient’s self-interest,principle of psychological support.In addition,suggestions on"system construction"from the perspective of medical ethics have were put forward to provide some references for the clinical practice of relevant medical professionals and the formulation of laws and regulations related to medical health in China.
