摘要
目的:探讨2型糖尿病(DM)患者血清细胞因子水平与发生糖尿病肾脏疾病(DKD)的相关性,并进一步分析DKD患者细胞因子水平与肾脏损伤程度的关系。方法:收集2019年2月~12月中日友好医院内分泌科和肾内科住院患者227例,单纯DM组131例,DKD组96例。DKD患者分为eGFR下降组(<60ml/min/1.73m2)和正常组(≥60ml/min/1.73m2)。采用化学发光法检测血清细胞因子:可溶性白介素2受体(SIL2R)、白介素8(IL8)和α肿瘤坏死因子(TNF-α)水平。分析各细胞因子在各组间的差异,并行相关性分析。结果:DKD组SIL2R、TNF-α水平明显高于DM组(均P<0.05)。DKD患者中eGFR下降组TNF-α、SIL2R水平明显高于eGFR正常组(均P<0.05)。Spearman相关性分析显示SIL2R、TNF-α与eGFR负相关,r值分别为-0.785和-0.444 (均P<0.05)。结论:DKD患者血清中TNF-α、SIL2R水平升高可能与其发生、发展正相关,提示TNF-α、SIL2R对于评估DKD的发病风险和病情进展具有一定意义。
Objective:To investigate the correlation between serum cytokine levels in patients with type 2 diabetes(DM)and Diabetic Kidney Disease(DKD),and to further analyze the correlation between cytokine levels and the degree of kidney damage in DKD patients.Methods:A total of 227 patients with type 2 DM(T2 DM)treated in the Department of Endocrinology and Nephrology in China-Japan Friendship Hospital from February to December 2019 were collected,131 in the T2 DM group and 96 in the DKD group.Serum cytokine soluble interleukin 2 receptor(SIL2 R),interleuki 8(IL8)and alpha tumor necrosis factor(TNF-α)levels were detected by chemiluminescence.The DKD patients were divided into two groups:eGFR <60 ml/min/1.73 m2 and eGFR ≥60 ml/min/1.73 m2.Correlations of SIL2 R,TNF-α and e GFR were analyzed by Spearman correlation analysis.Results:The levels of TNF-α and SIL2 R in the DKD group were significantly higher than those in the T2 DM group(P<0.05).The levels of TNF-α and SIL2 R in eGFR<60 ml/min/1.73 m2 group were significantly higher than those in ≥60 ml/min/1.73 m2 group(P<0.05).Spearman correlation analysis in the DKD group showed negative correlations between SIL2 R,TNF-α and e GFR(r=-0.785 and r=-0.444,P<0.05).Conclusion:There is a correlation between serum TNF-α and SIL2 R levels and the occurrence and development of DKD,suggesting that TNF-α and SIL2 R are useful in evaluating the incidence of DKD.
