探讨MRI对幕下毛细胞星形细胞瘤与室管膜瘤的诊断价值 被引量:2

摘要 目的对比观察幕下毛细胞星形细胞瘤与室管膜瘤的MRI表现,提高MRI的鉴别诊断水平。方法回顾性分析经病理证实的14例幕下毛细胞星形细胞瘤及11例室管膜瘤的MRI表现及临床资料,对比分析二者的内部信号、生长方式、是否有流空血管、瘤体囊性成分T2 FLAIR信号、实性成分DWI信号、增强特点的差异。结果幕下室管膜瘤部分有流空血管、而毛细胞星形细胞瘤无流空血管的特点差异有统计学意义(P<0.05);幕下毛细胞星形细胞瘤瘤体实性成分DWI信号呈低信号、室管膜瘤呈等信号差异具有统计学意义(P<0.01);幕下毛细胞星形细胞瘤MRI增强呈明显及显著强化、室管膜瘤呈轻中度强化差异有统计学意义(P<0.05)。结论幕下毛细胞星形细胞瘤与室管膜瘤的MRI表现具有一定的特征性,结合肿瘤是否有流空血管、DWI及强化特点可明显提高MRI诊断的准确率。 Objective To compare and observe the MRI features of subtentorial pilocytic astrocytoma and ependymoma,so as to improve the differential diagnosis level of MRI.Methods The MRI findings and clinical data of 14 cases of Subtentorial pilocytic astrocytoma and 11 cases of ependymoma confirmed by pathology were retrospectively analyzed.The difference in internal signal,growth pattern,the existence of flowing blood vessels,T2 FLAIR signal of cystic component,DWI signal of solid component and enhancement characteristics were compared and analyzed.Results Subtentorial ependymoma had some flowing blood vessels,but pilocytic astrocytoma had no characteristics,the difference was significant(P<0.05).Subtentorial pilocytic astrocytoma showed low signal intensity on DWI and ependymoma showed equal signal intensity which had obvious statistical significance(P<0.01).Subtentorial pilocytic astrocytoma showed marked and marked enhancement on MRI,and ependymoma showed mild to moderate signal intensity,which had statistical significance(P<0.05).Conclusion Subtentorial pilocytic astrocytoma and ependymoma have certain characteristics on MRI.And with the combination of the existence of flowing vessels,DWI and enhancement features,the accuracy of MRI can be significantly improved.
