骨科手术机器人在个体化椎间孔镜穿刺导向中的应用 被引量:12

摘要 目的椎间孔镜手术操作中穿刺是重中之重,应用“天玑”骨科手术机器人协助穿刺定位可有效降低学习曲线,现观察比较改进后二维方法与传统三维定位法的差异,以找出更适合临床的定位方法。方法为验证该项技术的安全性和有效性,选取60例腰椎间盘突出症患者,随机分成两组,A组机器人三维穿刺组30例,B组应用机器人二维穿刺组30例,分别对比两组穿刺成功率,透视次数、手术时间及术后疗效。结果在穿刺成功率和术后疗效方面,二者无显著差异(P>0.05);在透视次数和手术时间方面,二维组明显少于三维组(P<0.05)。结论改进后骨科机器人技术可以精准引导术中穿刺,减少透视次数,避免辐射损伤,手术时间明显缩短,且安全性和有效性与传统三维定位方法没有明显区别,值得临床推广应用。 Objective Puncture is the most important operation in the operation of PELD.Application of"Ti Robot"to assist puncture positioning can effectively reduce the learning curve.This study was designed to observe and compare the difference between the improved two-dimensional method and the traditional three-dimensional positioning method,in order to find a more suitable location method.Methods To verify the safety and effectiveness of this technique,60 patients with lumbar disc herniation were selected and randomly divided into two groups,with 30 cases in each group.Group A received robot three-dimensional puncture,and group B received robot two-dimensional puncture.The success rate of puncture,the times of fluoroscopy and the time of operation were compared between the two groups.Results There were no significant differences in the success rate of puncture and the clinical effect after operation(P>0.05).The times of fluoroscopy and operation time of group B were significantly less than those of group A(P<0.05).Conclusion The improved orthopedic robot technology can precisely guide the puncture in operation,reduce the times of fluoroscopy,avoid radiation injury,and shorten the operation time.And there are no obvious differences in the safety and effectiveness compared with the traditional three-dimensional positioning method,which is worthy of clinical application.
