摘要
目的分析无痛分娩不同体位对首次分娩产妇疼痛、产后出血及分娩方式的影响。方法选取平顶山市平煤神马医疗集团总医院于2017年1月—2018年12月间收治的首次分娩产妇100例,按双盲分组法分成卧位组和自由组,每组各50例。卧位组进行卧位待产,自由组选择自由体位。比较两组首次分娩产妇疼痛、产后出血、分娩方式及产程时间。结果两组首次分娩产妇在第一产程的视觉模拟评分(VAS)差异无统计学意义(P>0.05),自由组第二产程、第三产程VAS评分低于卧位组(P<0.05)。自由组首次分娩产妇产后2 h平均出血量、产后总出血量少于卧位组(P<0.05)。自由组首次分娩产妇自然分娩率高于卧位组(P<0.05),剖宫产率低于卧位组(P<0.05),阴道助产率低于卧位组(P>0.05)。自由组首次分娩产妇第一产程活跃期、第二产程及第三产程时间少于卧位组(P<0.05)。结论无痛分娩自由体位能减少首次分娩产妇疼痛、产后出血,便于产妇自然分娩,减少剖宫产,缩短产程时间,推荐使用。
Objective To investigate the effect of different painless delivery positions on pain,postpartum hemorrhage and delivery methods in primipara.Methods 100 primiparas in the hospital from January,2017 to December,2018 were enrolled,and divided into two groups by double-blind grouping method,with 50 cases in each group.The supine group was in the supine position while the free group was in the free position.Then the pain degree,postpartum hemorrhage,delivery methods and labor process were compared between groups.Results There was no difference in VAS score in the first labor stage between two groups(P>0.05),while the scores in the second and third labor stages were lower in free group than in supine group(P<0.05).The average postpartum hemorrhage and total postpartum hemorrhage 2 h after delivery of free group were less than those in supine group(P<0.05).Free group had higher natural delivery rate(P<0.05),lower cesarean delivery rate(P<0.05),and lower vaginal delivery rate(P>0.05)than those of supine group.The duration of the first,second,and third labor stages of free group was shorter than that of supine group(P<0.05).Conclusion Primiparas undergoing painless delivery in free position can effectively reduce pain degree and postpartum hemorrhage,improve the natural delivery rate,reduce the cesarean section,shorten the labor process time,which is recommended.
