摘要
目的研究硬膜外阻滞麻醉技术在分娩镇痛中的应用价值。方法以2017年3月至2018年6月接诊的足月单胎初产妇100例为研究对象,采用电脑随机双盲法分成试验组和对照2组,每组50例。试验组接受分娩镇痛,并在分娩期间应用硬膜外阻滞麻醉技术,对照组接受自然分娩。分析2组的产程进展情况,并对2组的产后2 h出血量以及镇痛效果等指标作出比较。结果试验组的第一产程时间为(7.79±1.02)h,比对照组的(9.83±1.24)h短(P<0.05)。2组的第二和第三产程时间比较差异无统计学意义(P>0.05)。试验组的自然分娩率为94.0%、镇痛有效率为96.0%,比对照组的70.0%、2.0%高(P<0.05)。试验组的产后2 h出血量同对照组比较差异无统计学意义(P>0.05)。结论将硬膜外阻滞麻醉技术合理运用于无痛分娩中,不仅有助于减轻分娩疼痛,同时还能有效缩短第一产程时间,提高自然分娩成功率,且不会增加产妇的产后2 h出血量,十分安全有效,建议临床推广。
Objective To explore the application value of epidural block anesthesia in painless delivery.Methods A total of 100 single birth term primipara, who were diagnosed in our hospital from March 2017 to June 2018, were randomly divided into experimental group(n=50) and control group(n=50). The primipara in experimental group received painless delivery and underwent epidural block anesthesia during delivery,however, the primipara in control group received natural delivery. The labor processes, blood loss at 2 h after delivery and analgesia effects were observed and compared between the two groups.Results The time of the first stage of labor in experimental group(7.79±1.02) was significantly shorter than that in control group(9.83±1.24)h.There were no significant differences in the time of the second stage of labor and the time of the second stage of labor between the two groups(P>0.05). Moreover, the natural delivery rate and the effective rate of pain relief in experimental group were 94.0% and 96.0%, respectively, which were significantly higher than those in control group(70.0%, 2.0%,respectively)(P<0.05). In addition, there was no significant difference in blood loss at 2 h after delivery between the two groups(P>0.05).Conclusion The epidural block anesthesia is safe and effective,which can not only helps to relieve pain, but also effectively shorten the first labor process, improve the success rate of natural labor and will not increase the blood loss at 2 h after delivery,therefor, it is worthy of popularization in clinical practice.
