摘要
目的观察腺苷蛋氨酸(SAMe)治疗胆汁淤积性肝病(CSLD)的效果及对患者肝功能和血清磷脂转运蛋白(PLTP)、白细胞介素-18(IL-18)水平的影响。方法选择焦作市人民医院2017年5月至2019年3月诊治的CSLD患者92例,根据患者自愿以及随机原则分为对照组和研究组,每组46例。两组均给予常规治疗,研究组在此基础上给予SAMe治疗,均治疗4周。比较两组患者治疗前后肝功能指标和血清PLTP、IL-18水平,并评估治疗效果、患者临床症状变化情况以及安全性。结果治疗后,研究组总胆汁酸、丙氨酸氨基转移酶、总胆红素、血清PLTP和IL-18水平以及瘙痒症状评分均低于对照组,白蛋白水平高于对照组(P<0.05)。研究组治疗总有效率(93.48%,43/46)优于对照组(78.26%,36/46),P<0.05。两组不良反应情况比较差异未见统计学意义(P>0.05)。结论SAMe治疗CSLD患者安全有效,可有效改善患者临床症状,促进患者肝功能恢复,降低血清PLTP、IL-18水平。
Objective To study the curative effects of S-adenosylmethionine(SAMe)on cholestatic liver disease(CSLD)and its influence on liver function,levels of serum phospholipid transfer protein(PLTP)and interleukin-18(IL-18).Methods A total of 92 CSLD patients who were treated in Jiaozuo People’s Hospital from May 2017 to March 2019 were enrolled.According to voluntary and random principle,selected patients were divided into control group and study group,with 46 cases in each group.Both groups were given conventional treatment,while the study group was given SAMe treatment additionally.Both groups were treated for 4 weeks.The liver function indexes and levels of serum PLTP and IL-18 before and after treatment were compared between the two groups.The curative effect,changes of clinical symptoms and safety of treatment were evaluated.Results After treatment,levels of total bile acid(TBA),alanine aminotransferase(ALT),total bilirubin(TBIL),serum PLTP,IL-18,and scores of pruritus symptom in study group were lower than those in control group,while albumin level was significantly higher than that in control group(P<0.05).The total effective rate of the study group(93.48%,43/46)was better than that of the control group(78.26%,36/46),P<0.05.There were no significant differences in adverse reactions between the two groups(P>0.05).Conclusions SAMe is safe and effective in the treatments of CSLD patients,which can effectively improve their clinical symptoms,promote liver function recovery,and reduce levels of serum PLTP and IL-18.
