摘要
目的观察数字化跑台训练改善脑卒中患者步行能力和下肢功能的效果,并探讨其临床意义。方法选择脑卒中患者40例,其中男性29例,女性11例,年龄30~75岁,平均年龄52.70岁;病程1~11个月,平均病程3.67个月;脑出血22例,脑梗死18例。随机分为对照组和治疗组,每组各20例。对照组给予常规康复治疗,治疗组给予常规康复治疗及数字化跑台训练。分别于治疗前和治疗2周后测量患者双侧平均步长、步行时双侧髋、膝关节最大屈曲角度。同时进行下肢Fugl-Meyer运动功能评定(FMA-L)和10 m步行时间评估。结果治疗2周后,两组患者双侧平均步长较治疗前提高[治疗组健侧(0.42±0.05)m vs(0.25±0.04)m,患侧(0.48±0.04)m vs(0.41±0.04)m;对照组健侧(0.38±0.06)m vs(0.26±0.05)m,患侧(0.45±0.04)m vs(0.42±0.05)m],差异有显著统计学意义(P<0.001);且治疗组与对照组相比,差异有统计学意义(P<0.05)。两组患者步行时双侧髋、膝关节最大屈曲角度和FMA-L较治疗前提高(P<0.001),且治疗组均优于对照组(P<0.05)。两组患者10 m步行时间较治疗前缩短[治疗组(26.02±7.05)s vs(31.10±7.75)s;对照组(30.86±6.72)s vs(32.17±6.90)s](P<0.001),且治疗组优于对照组(P<0.05)。结论数字化跑台训练可以有效改善脑卒中患者下肢的运动功能和步行能力。
Objective To observe the effects of digital treadmill training on walking ability in stroke patient.Methods A total of 40 stroke patients were enrolled,which included 29 males and 11 females,aged 30-75 years old with mean age of52.70 years old.The disease course was 1-11 months with mean course of 3.67 months;22 cases with cerebral hemorrhage and 18 with cerebral infarction.All of them were randomized into control group(conventional rehabilitation treatment)and treat-ment group(conventional rehabilitation and digital treadmill training).The bilateral mean step length,maximum flexion angle of bilateral hip and knee joints during walking before treatment and after treatment 2-week were measured.The lower limb Fugl-Meyer Assessment(FMA-L)and 10-meter walking time were evaluated.Results After treatment 2-week,the bilateral mean step length of 2 groups were higher than before treatment[treatment group:healthy side(0.42±0.05)meters vs(0.25±0.04)meters,affected side(0.48±0.04)meters vs(0.41±0.04)meters;control group:healthy side(0.38±0.06)meters vs(0.26±0.05)meters,affected side(0.45±0.04)meters vs(0.42±0.05)meters],and the difference was statistically significant(P<0.001);the difference was statistically significant between control group and treatment group(P<0.05).The maximum flexion angle and FMA-L of bilateral hip and knee joints of 2 groups were higher than before treatment(P<0.001),and treatment group was better than those of control group(P<0.05).The 10-meter walking time of 2 groups was lower than before treatment[treatment group:(26.02±7.05)seconds vs(31.10±7.75)seconds;control group:(30.86±6.72)seconds vs(32.17±6.90)seconds](P<0.001),and treatment group was better than control group(P<0.05).Conclusion It is demonstrated that digital treadmill training could improve the lower limbs motor function and walking ability in stroke patient.
