摘要 目的探讨胃神经鞘瘤(GS)的CT影像特点,提高对GS的认识及诊断准确率。方法回顾性分析经过手术病理证实的11例GS的CT表现,总结其影像学特点。结果11例均为单发病灶,10例位于胃体,1例位于胃窦。10例呈圆形或卵圆形,1例呈浅分叶状。8例向腔外生长,2例向腔内、外生长,1例向腔内生长。9例平扫密度均匀;2例密度不均匀,其中1例囊变及溃疡,1例边缘点状钙化。增强扫描10例动脉期轻度强化,门静脉期中等度强化,平衡期呈明显强化。5例患者伴周围淋巴结肿大。结论GS有一定的CT影像学特点,多位于胃体,平扫密度均匀,坏死与钙化少见,增强扫描具有渐进性强化的特征,CT可以对GS做出较准确的诊断。 Objective To explore the imaging features of gastric schwannoma(GS)in order to improve understanding and accuracy diagnosis of GS.Methods The CT findings of 11 gastric schwannomas proved by operation and pathology were retrospectively analyzed and the imaging features were summarized.Results All the 11 cases were single lesions,10 masses were located in the gastric body and 1 mass was located in the gastric antrum.10 masses showed morphologically as round or ovoid shape,and one mass was superficial and lobulated.For their growth pattern,8 masses grew outside the cavity fields,2 masses presented as bilateral growth and 1 mass intra-cavitary growth.9 masses appeared uniform density,2 tumors showed inhomogeneous density,1 masse appeared cystic change and ulcer and 1 masse had dotted calcification.10 masses showed mild enhancement on the artery phase of enhanced scanning,medium enhancement on portal phase and obvious enhancement on balanced period.5 masses were surrounded by swelling lymph nodes.Conclusion GS has some certain characteristics on CT.The masses were mostly located in gastric body and homogeneous density.Necrosis and calcification are rare in the mass.Progressive enhancement is a characteristic findings of mass on dynamic enhanced CT scanning.CT is helpful for the accuracy diagnosis of GS.
