摘要
目的探讨PFNA内固定治疗对老年股骨粗隆间骨折患者髋关节功能及并发症的影响。方法选取2014年2月至2019年7月江铜集团东铜医院及江西省人民医院骨科收治的100例老年股骨粗隆间骨折患者作为研究对象,根据随机双盲法分为对照组和实验组,每组50例。对照组采用动力髋螺钉(DHS)治疗,实验组采用PFNA治疗。比较两组手术相关指标、髋关节功能及术后并发症发生情况。结果实验组手术相关指标(手术时间、术中出血量、术后引流量)均低于对照组,差异均有统计学意义(P<0.05);术后实验组髋关节疼痛、行走、活动度、功能评分及总分均高于对照组,且并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论 PFNA内固定手术治疗老年股骨粗隆间骨折可有效改善髋关节功能,安全性高,并发症发生率低,临床效果显著,值得临床推广应用。
Objective The effect of internal fixation of PFNA on the hip function and complications of the elderly patients with intertrochanteric fracture. Methods 100 cases old intertrochanteric fractures of the femoral intertrochanteric fracture in the East Copper Hospital of Jiangcopper Group and the People’ s Hospital of Jiangxi Province from February 2014 to July 2019 were selected as the study subject. They were divided into the control group and the experimental group according to the randomized double-blind method, with 50 cases in each group. The control group was treated with dynamic hip screw(DHS) and the experimental group was treated with PFNA. The operative parameters, hip function and postoperative complications were compared between the two groups. Results The operation-related indexes(operation time, intraoperative blood loss, postoperative drainage volume) of the experimental group were higher than those of the control group(P<0.05). The postoperative hip pain, walking, range of motion, function and total score of the experimental group were higher than those of the control group(P<0.05), and the incidence of complications was lower than that of the control group(P<0.05). Conclusion PFNA internal fixation can effectively improve the function of hip joint, the safety is high, the incidence of complications is low, and the clinical effect is remarkable, it is worthy of clinical application.
