摘要
目的探讨多普勒超声引导下痔动脉结扎治疗痔疮的效果。方法选取该院于2017年12月—2019年5月收诊的76例痔疮患者,等距抽样法分为观察组(多普勒超声引导痔动脉结扎术,38例)与对照组(传统痣上黏膜环切术,38例)2组,比较2组痔疮患者的下床活动天数、便血消失天数,术后排便天数及并发症发生情况。结果观察组痔疮患者手术后的下床活动天数(1.25±0.18)d、便血消失天数(0.62±0.13)d、术后排便天数(2.05±0.28)d,对照组手术后的下床活动天数(3.04±0.31)d、便血消失天数(2.27±0.36)d、术后排便天数(3.34±0.46)d,组间比较差异有统计学意义(P<0.05);观察组发生尿潴留1例(2.63%),出血1例(2.63%),并发症发生率5.26%;对照组发生尿潴留3例(7.89%),出血4例(10.53%),严重疼痛2例(5.26%),并发症发生率为23.68%,观察组并发症的发生率低于对照组,差异有统计学意义(χ~2=5.208,P=0.022<0.05)。观察组患者Wexner评分为(2.14±0.12)分,对照组为(5.73±0.33)分,观察组的Wexner评分低于对照组,差异有统计学意义(t=63.023,P=0.000<0.05)。结论多普勒超声引导痔动脉结扎术治疗痔疮患者效果显著,能减轻术后便血,利于早期下床活动、术后排便,降低并发症的发生。
Objective To investigate the effect of hemorrhoids treated with hemorrhoid artery ligation under Doppler ultrasound guidance.Methods 76 patients with hemorrhoids admitted in the hospital from December 2017 to May 2019 were divided into observation group(Doppler ultrasound-guided hemorrhoid artery ligation,38 cases)and control group(Traditional circummucous resection of nevus,38 cases),compared the number of days of getting out of bed,the time of disappearance of blood in the stool,the number of postoperative defecation days and the occurrence of complications in the two groups.Results Patients in the observation group had out-of-bed movement days(1.25±0.18)d,blood loss in the stool(0.62±0.13)d,postoperative defecation days(2.05±0.28)d in the observation group,and out-of-bed activities days in the control group(3.04±0.31)d,days of disappearance of blood in the stool(2.27±0.36)d,days of postoperative defecation(3.34±0.46)d,the difference between groups was statistically significant(P<0.05);1 case of urinary retention occurred in the observation group(2.63%),1 case of bleeding(2.63%),complication rate of 5.26%;3 cases of urinary retention(7.89%),4 cases of bleeding(10.53%),2 cases of severe pain(5.26%),complications occurrence rate was 23.68%in the control group,and the incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(χ2=5.208,P=0.022<0.05).The Wexner score of patients in the observation group was(2.14±0.12)points,and the control group was(5.73±0.33)points.The Wexner score in the observation group was lower than that in the control group,the difference was statistically significant(t=63.023,P=0.000<0.05).Conclusion Doppler ultrasound-guided hemorrhoid artery ligation is effective in treating hemorrhoid patients.It can reduce postoperative blood in the stool,facilitate early getting out of bed,postoperative defecation,and reduce complications.
