摘要
目的:调查日间病房白内障手术患者的出院指导质量、出院准备度及出院后健康结局现状,并分析三者之间的关联。方法:采用横断面研究设计,采用方便抽样法于2017年10月至2018年1月选取来自四川省某三级甲等医院眼科日间病房的194位白内障手术患者。采用自行设计的一般资料调查表、出院指导质量量表、出院准备度量表和出院后健康结局调查表进行调查。结果:共194名患者完成了本研究,185名(95.36%)患者在出院当天表示已准备好出院。患者的出院指导质量、出院准备度及出院后健康结局量表得分中位数分别为196.00分、179.50分和78.00分。出院后两周,患者术眼视力的中位数为0.80,且均未发生并发症,但有36人(18.56%)在出院后出现术眼不适症状。患者的出院指导质量与出院准备度呈正相关,相关系数为0.59(P<0.001);患者的出院准备度与出院后健康结局呈正相关,相关系数为0.27(P<0.001);患者的出院指导质量与出院后健康结局呈正相关,相关系数为0.25(P=0.001)。结论:白内障日间手术患者的出院指导质量、出院准备度及出院后健康结局较好。但医务人员为患者提供的出院指导内容还不能完全满足其需求,医务人员应更关注患者的个体化出院指导需求,并注重评估患者对指导内容的理解情况,同时向其强调遵医行为的重要性,以更好地帮助患者预防并发症发生和促进术后康复。
Objective:To investigate the quality of discharge teaching service,readiness for hospital discharge,and postdischarge health outcomes of cataract patients in the day surgery ward,and to explore the relationships among these variables.Methods:This cross-sectional study design employed a convenience sample of 194 cataract patients from the ophthalmic day surgery ward in a tertiary level general hospital in Sichuan province from October 2017 to January 2018.The general information questionnaire,Quality of Discharge Teaching Scale(QDTS),Readiness for Hospital Discharge Scale(RHDS)and Post-discharge Health Outcome Questionnaire were used for the survey.Results:A total of 194 patients completed the study,and 185(95.40%)patients answered that they had been ready to leave the hospital on the day of discharge.The median score of quality of discharge teaching service,readiness for hospital discharge,and post-discharge health outcomes were 196.00,179.50 and 78.00,respectively.Two weeks after discharge,the median visual acuity of the patients was 0.80,and no complications occurred.However,36(18.56%)patients had eye discomfort.It was found that the score of quality of discharge teaching service was positively correlated with the score of readiness for hospital discharge(r=0.59,P<0.001).The score of readiness for hospital discharge had a positive correlation with the score of post-discharge health outcomes(r=0.27,P<0.001),and the score of quality of discharge teaching service was also correlated with the score of post-discharge health outcomes(r=0.25,P=0.001).Conclusion:Cataract patients in the day surgery ward receive good quality of discharge teaching service.Their readiness for discharge and post-discharge health outcomes are good,too.However,the discharge teaching content provided to patients couldn't fully meet their needs.Therefore,we as medical workers should pay more attention to the patients'individualized needs of discharge teaching service,and evaluate their understanding of the discharge teaching content.Meanwhile,
