摘要
目的探讨三叉神经磁共振成像(MRI)检查中三维时间飞跃(3D-TOF)、三维稳态进动快速成像(3D-FIESTA)序列二维图像及经单模配准重建后所得到的三维融合图像对于原发性三叉神经痛及偏侧面肌痉挛的诊断价值。方法回顾性分析73例行三叉神经磁共振断层血管成像(MRTA)检查的患者(其中原发性三叉神经痛48例、偏侧面肌痉挛25例)的二维图像与三维图像2组图像,分别对三叉神经、面神经邻近血管位置关系做出诊断。以手术结果为金标准,计算2组图像对症状侧神经与血管关系诊断的敏感度。结果48例原发性三叉神经痛患者中,三维图像诊断的敏感度高于二维图像的敏感度(93.8%vs.87.5%,P<0.05)。25例偏侧面肌痉挛患者中,三维图像诊断的敏感度高于二维图像的敏感度(92.0%vs.84.0%,P<0.05)。结论与3D-TOF和3D-FIESTA序列二维图像相比,3D-TOF与3D-FIESTA序列融合后的三维图像能更清楚地显示神经与血管的位置关系,对原发性三叉神经痛及偏侧面肌痉挛有较高的诊断价值。
Objective To explore the diagnostic value of 3D time of flight(3D-TOF)and 3D fast imaging employing steady-state acquisition(3D-FIESTA)sequence two dimensional images and three-dimensional fusion images after single-mode registration and postreconstruction during magnetic resonance imaging(MRI)of trigeminal nerve for the diagnosis of primary trigeminal neuralgia and hemifacial spasm.Methods The two-dimensional and three-dimensional images of 73 patients(48 cases of primary trigeminal neuralgia and 25 cases of hemifacial spasm)who were examined by magnetic resonance tomographic angiography(MRTA)were analyzed retrospectively.The relationship of location between the trigeminal nerve and the adjacent blood vessels and the facial nerve and the adjacent blood vessels was diagnosed respectively.Using the operation result as the gold standard,the sensitivity of the two groups of images to the diagnosis of the relationship between the symptomatic nerves and their blood vessels was calculated.Results In 48 patients with primary trigeminal neuralgia,the diagnostic sensitivity of three-dimensional images was higher than that of two-dimensional images(93.8%vs.87.5%,P<0.05).In 25 patients with hemifacial spasm,the sensitivity of three-dimensional images was higher than that of two-dimensional images(92.0%vs.84.0%,P<0.05).Conclusions Compared with two-dimensional images of 3D-TOF and 3D-FIESTA,three-dimensional fusion images of 3D-TOF and 3D-FIESTA can clearly show the relationship of location between nerve and blood vessel,which has higher diagnostic value for primary trigeminal neuralgia and hemifacial spasm.
