摘要
目的通过对社区甲状腺结节患者的长期监测与随访,加强社区对甲状腺结节患者的有效管理。方法选取2017年6月—2018年2月在上海市浦东新区金杨社区卫生服务中心全科门诊就诊,并经超声检查新发现并确诊为甲状腺结节的患者145例,进行为期12个月的随访管理。结果随访12个月,145例甲状腺结节患者单发与多发、大小的变化,差异有统计学意义(P<0.001);TSH的变化,差异无统计学意义(P>0.05);1例患者随访中结节直径增加超过50.00%,转院手术治疗,术后病理报告诊断为甲状腺乳头状癌,占管理人数的0.69%。管理12个月前后比较,患者对甲状腺结节及相关知识掌握程度明显提高,差异有统计学意义(P<0.001)。结论加强社区对甲状腺结节长期的监测、随访和管理,适当地转诊,能让甲状腺肿瘤等甲状腺疾病早发现、早诊断和早治疗。
OBJECTIVE Through long-term observation and follow-up of patients with thyroid nodules in the community,strengthening the effective management of thyroid nodule in the community.METHODS From June 2017 to February 2018,a total of 145 cases of newly diagnosed patients with thyroid nodules by thyroid ultrasonography were recently found out and selected in Jinyang Community Health Service Center.A total of 12 months of follow-up was performed.RESULTS After 12 months of follow up,changes of multiple,size in 145 patients with thyroid nodules,the difference was statistically significant(P<0.001),changes of TSH,the difference was not statistically significant(P>0.05).1 patient had a thyroid nodule increased by more than 50%,who was transferred to the hospital for surgical treatment.Postoperative pathological reports showed papillary thyroid carcinoma accounted for 0.69%of the management people.Before and after 12 months of follow-up management process,the patient’s knowledge about the health knowledge of thyroid nodules was significantly improved,and the difference was statistically significant(P<0.001).CONCLUSION It needed to strengthen long-term monitoring,follow-up and management and proper referral of thyroid nodules in the community to enable early detection,diagnosis and treatment of thyroid diseases such as thyroid carcinoma.
