头颅CTA联合MRA对短暂性脑缺血后卒中的预测价值 被引量:1

Value of cranial CTA combined with MRA in prediction of transient ischemic stroke

摘要 目的观察短暂性脑缺血患者的头颅CT血管造影(CTA)、磁共振血管造影(MRA)检出结果,分析头颅CTA与MRA联合用于缺血后卒中预测的价值。方法抽取2017年3月至2019年4月新乡市第一人民医院收治的101例短暂性脑缺血患者,全部患者在入院时均接受头颅CTA与MRA检查,观察患者入院时的颅内动脉狭窄情况,参照相关标准,记录患者缺血后卒中发生情况,绘制受试者工作曲线(ROC)得到曲线下面积(AUC),检验各影像学方法单独与联合检查用于预测短暂性脑缺血后卒中的价值。结果101例患者在住院治疗1周内经诊断发生缺血后卒中15例;入院当天,经CTA、MRA结果显示,住院7 d内发生卒中的短暂性脑缺血患者颈动脉狭窄程度、颈动脉狭窄率均高于未发生卒中的患者,差异均有统计学意义(P均<0.05);CTA、MRA单独与联合检出狭窄患者的卒中发生率高于未检出狭窄者,差异有统计学意义(P<0.05);绘制ROC结果显示,CTA、MRA单独联合预测短暂性脑缺血后卒中发生的AUC分别为0.875、0.897、0.900,均>0.850,预测价值均理想,且以联合预测的AUC值最高,较单独预测价值高。结论入院时头颅CTA与MRA检测短暂性脑缺血患者动脉狭窄情况可以用于缺血后卒中风险预测,尤以联合预测价值更高。 Objective To observe the results of cranial CT angiography(CTA)and magnetic resonance angiography(MRA)in patients with transient cerebral ischemia,and analyze the predictive value of cranial CTA combined with MRA in ischemic stroke.Methods A total of 101 patients with transient cerebral ischemia who admitted in the First People’s Hospital of Xinxiang from March 2017 to April 2019 were included.All patients underwent cranial CTA and MRA examination on admission.The intracranial arterial stenosis of patients on admission was observed.According to the relevant standards,the incidence of ischemic stroke was recorded,and the receiver operating characteristic curve(ROC)was drawn to obtain the area under the curve.The value of each imaging method alone and in combination for predicting transient ischemic stroke was examined.Results Fifteen cases of ischemic stroke were diagnosed in 101 patients within 1 week of hospitalization;on the day of admission,the results of CTA and MRA showed that the degree of carotid artery stenosis and the rate of carotid artery stenosis within 7 d of hospitalization in patients with transient cerebral ischemia were higher than those patients without stroke,and the differences were statistically significant(P all<0.05);the incidence of stroke in patients with stenosis alone and in combination with CTA and MRA was higher than that in patients without stenosis,and the difference was statistically significant(P<0.05);the ROC curve was drawn,and the results showed that the area under the curve(AUC)of CTA and MRA alone and in combination for predicting the incidence of transient ischemic stroke was 0.875,0.897,and 0.900,respectively,all of them were more than 0.850,and the prediction value was ideal,and the AUC value of combined prediction was the highest,which was higher than single prediction.Conclusions On admission,CTA and MRA detection of arterial stenosis in patients with transient cerebral ischemic can be used to predict the risk of ischemic stroke,and the value of combined prediction is
