摘要
目的观察彩色多普勒超声诊断创伤骨科急性下肢深静脉血栓(DVT)的效果。方法选择2018年8月至2019年5月于本院治疗的疑为急性DVT患者80例,入选者均接受彩色多普勒超声、深静脉造影检查。观察彩色多普勒超声检查结果,并以深静脉造影检查结果作为金标准,计算彩色多普勒超声诊断准确率、敏感度、特异度。结果本研究中疑为急性DVT的80例患者经深静脉造影诊断阳性、阴性分别为63例、17例,彩色多普勒超声诊断阳性、阴性分别为59例、21例;以深静脉造影诊断结果作为金标准,彩色多普勒超声诊断准确率、敏感度、特异度分别为90.00%(72/80)、90.48%(57/63)、88.24%(15/17);彩色多普勒超声、深静脉造影行一致性检验Kappa值为0.725,两者影像学方法诊断一致性较高;与健侧相比,患侧腘静脉、股浅静脉、股总静脉管腔内径相对较大,彩色多普勒血流(CDFI)相对较小,差异有统计学意义(P<0.05)。结论彩色多普勒超声为创伤骨科急性DVT诊断较为准确方法,有利于观察下肢各静脉血管内径及血流情况,为DVT诊断提供帮助。
Objective To observe the effect of color Doppler ultrasound in the diagnosis of acute lower extremity deep venous thrombosis(DVT) in traumatic orthopedics. Methods Eighty patients with suspected acute DVT who were treated in our hospital from August 2018 to May2019 were enrolled. All patients were enrolled in color Doppler ultrasound and deep vein angiography. The results of color Doppler ultrasonography were observed, and the results of deep vein venography were used as the gold standard to calculate the accuracy, sensitivity and specificity of color Doppler ultrasound diagnosis. Results In this study, 80 patients with suspected acute DVT were diagnosed positive and negative by deep venography in 63 cases and 17 cases respectively. Color Doppler ultrasound was positive and negative were 59 cases and 21 cases respectively. Diagnosis by deep vein angiography. As the gold standard, the accuracy, sensitivity and specificity of color Doppler ultrasound diagnosis were 90.00%(72/80),90.48%(57/63), 88.24%(15/17), respectively. Color Doppler ultrasound the Kappa value of the consistency test of deep venography was 0.725. The diagnostic consistency of the imaging technique was higher. Compared with the healthy side, the diameter of the iliac vein, superficial femoral vein and common femoral vein was relatively large. Doppler blood flow(CDFI) was relatively small, and the difference was statistically significant(P<0.05). Conclusion Color Doppler ultrasound is an accurate method for the diagnosis of acute DVT in traumatic orthopedics. It is helpful to observe the diameter and blood flow of venous vessels in lower extremities and provide assistance for the diagnosis of DVT.
