摘要
目的探讨血清脂蛋白相关磷脂酶A2(Lp-PLA2)、同型半胱氨酸(Hcy)及S100β对缺血性脑卒中后血管性痴呆(VD)的预测价值.方法选取2016年3月至2018年10月本院收治的200例缺血性脑卒中患者,选择同期健康体检者30例作为对照组,检测血清Lp-PLA2、Hcy及S100β水平,缺血性脑卒中患者3个月后依据是否并发VD分为VD组及NVD组,比较各组血清Lp-PLA2、Hcy及S100β水平差异,依据MMSE评分将VD组患者分为轻度、中度及重度,比较不同程度VD患者血清Lp-PLA2、Hcy、S100水平差异,采用pearson检验分析VD患者血清Lp-PLA2、Hcy及S100β与MMSE评分的相关性,绘制ROC曲线,分析Lp-PLA2、Hcy及S100β对缺血性脑卒中后VD的预测价值.结果3组患者的血清Lp-PLA2、Hcy、S100β比较结果:VD组>NVD组>对照组,差异有统计学意义(P<0.05);不同认知障碍VD患者血清Lp-PLA2、Hcy和S100β比较结果:重度>中度>轻度,差异有统计学意义(P<0.05);VD患者的血清Lp-PLA2(r=-0.547,P=0.000)、Hcy(r=-0.511,P=0.000)、S100β(r=-0.628,P=0.000)与MMSE评分均呈显著的负相关关系,差异有统计学意义(P<0.05);Lp-PLA2、Hcy和S100β水平单独检测预测缺血性脑卒中后VD的AUC值为0.726、0.768、0.794,联合检测预测缺血性脑卒中后VD的AUC值为0.892.结论缺血性脑卒中患者血清血清Lp-PLA2、Hcy和S100β水平升高,并且与VD的严重程度相关,联合检测Lp-PLA2、Hcy和S100β对缺血性脑卒中后VD具有较好的预测价值.
Objective To explore the predictive value of Lp-PLA2,Hcy and S100βin vascular dementia(VD)after ischemic stroke.Methods 200 cases of patients with ischemic stroke in our hospital were selected from Mar 2016 to Oct 2018,and 30 healthy people in the same period were selected as the control group.levels of Lp-PLA2,Hcy and S100βwere measured in serum.Patients with ischemic stroke were divided into the VD group and the NVD group according to whether VD was complicated after months,levels of Lp-PLA2,Hcy and S100βin serum were measured in each group,patients in the VD group were divided into mild,moderate and severe condition according to MMSE score,levels of Lp-PLA2,Hcy and S100 in VD patients were compared in different condition.Pearson test was used to analyze correlation Lp-PLA2,Hcy and S100βwith MMSE score in VD patients.The ROC curve was drawn to analyze the predictive value of LpPLA2,Hcy and S100βfor VD after ischemic stroke.Results The results of serum Lp-PLA2,Hcy,and S100βin the three groups of patients:the VD group>the NVD group>the control group.The difference was statistically significant(P<0.05).Comparison results of serum Lp-PLA2,Hcy and S100βin patients with different cognitive impairment VD:severe>moderate>mild,the difference was statistically significant(P<0.05).Serum Lp-PLA2(r=-0.547,P=0.000),Hcy(r=-0.511,P=0.000),S100β(r=-0.628,P=0.000)of VD patients were significantly negatively correlated with MMSE scores.The difference was statistically significant(P<0.05).The Lp-PLA2,Hcy and S100βlevels were detected separately to predict the AUC value of VD after ischemic stroke were 0.726,0.768,0.794,and the combined detection predicted the AUC value of VD after ischemic stroke was 0.892.Conclusions Serum Lp-PLA2,Hcy and S100βlevels are elevated in patients with ischemic stroke and related to the severity of VD.Joint detection of Lp-PLA2,Hcy and S100βhas a good predictive value for VD after ischemic stroke.
