摘要
目的静脉血栓是骨折后常见并发症,易引起静脉瓣膜功能不全和肺静脉堵塞等,危害患者生命安全,而机体创伤应激所致血液滞缓和高凝状态是静脉血栓形成重要原因,本研究探讨凝血四项、血浆纤维蛋白原降解产物(fibrinogen degradation product,FDP)和D-二聚体(D-dimer,DD)检测在骨折中临床价值。方法选取2017-04-01-2019-05-31本院收治的135例骨折患者作为病例组,另选同期健康体检者80名作为对照组。均检测凝血酶原时间(prothrombin time,PT)、凝血酶时间(thrombin time,TT)、纤维蛋白原(fibrinogen,Fg)、活化部分凝血酶原时间(activated partial thromboplastin time,APTT)、FDP和DD水平,对比两组及有无静脉血栓骨折患者上述指标水平。结果病例组Fg为(3.99±0.35)g/L,高于对照组的(2.76±0.39)g/L,t=23.861,P<0.001;病例组FDP为(1 934.86±457.45)μg/L,高于对照组的(397.65±130.72)μg/L,t=29.330,P<0.001;DD为(430.92±57.51)μg/L,高于对照组的(209.70±47.39)μg/L,t=29.047,P<0.001。病例组静脉血栓患者Fg为(4.61±0.30)g/L,高于无静脉血栓患者的(3.58±0.40)g/L,t=16.131,P<0.001;FDP为(2 290.81±437.86)μg/L,高于无静脉血栓患者的(1 697.56±478.54)μg/L,t=7.297,P<0.001;DD为(546.05±59.87)μg/L水平,高于无静脉血栓患者的(354.16±56.38)μg/L,t=18.899,P<0.001。结论骨折患者并发静脉血栓时Fg、FDP、DD水平会显著升高,临床可根据其水平变化早期评估静脉血栓发生风险,以针对性干预,改善预后。
OBJECTIVE Venous thrombosis is a common complication after fracture,which can easily cause venous valve insufficiency and pulmonary vein occlusion,which endangers the life safety of patients.Blood stasis and hypercoagulability caused by trauma stress in the body are important causes of venous thrombosis.This study explores the clinical value of four items of blood coagulation,fibrinogen degradation product(FDP)and D-dimer(DD)detection in fractures.METHODS A total of 135 fracture patients admitted in our hospital from April 01,2017 to May 31,2017 were selected as the case group,and 80 healthy people who were examined during the same period were selected as the control group.The level of prothrombin time(PT),thrombin time(TT),fibrinogen(Fg),activated partial thromboplastin time(APTT),FDP and DD of the above indicators were tested and compared between the two groups and patients with or without venous thromboembolic fractures.RESULTS The Fg of the case group was(3.99±0.35)g/L,which was higher than that of the control group(2.76±0.39)g/L,t=23.861,P<0.001.The FDP of the case group was(1 934.86±457.45)μg/L,which was higher than that of the control group(397.65±130.72)μg/L,t=29.330,P<0.001;DD was(430.92±57.51)μg/L,which was higher than that of the control group(209.70±47.39)μg/L,t=29.047,P<0.001.The Fg of patients with venous thrombosis in the case group was(4.61±0.30)g/L,which was higher than that of patients without venous thrombosis(3.58±0.40)g/L,t=16.131,P<0.001.FDP was(2 290.81±437.86)μg/L,which was higher than that of patients without venous thrombosis(1 697.56±478.54)μg/L,t=7.297,P<0.001.DD was(546.05±59.87)μg/L,which was higher than that of patients without venous thrombosis(354.16±56.38)μg/L,t=18.899,P<0.001.CONCLUSIONS The level of Fg,FDP and DD in patients with fractures complicated by venous thrombosis will significantly increase.Clinically,the risk of venous thrombosis can be assessed early in the clinic based on changes in their levels,to target intervention and improve the prognosis.
