摘要
目的调查山西省临汾市国家级贫困县医院临床护士核心能力的现状以及其影响因素,旨在为护理管理者建立护士培养方案提供参考依据。方法采用便利抽样法,于2018年11月1—30日,以山西省临汾市国家级贫困县医院中的3家医院(A、B、C)的临床注册护士作为研究对象。采用自制的一般资料问卷和中国注册护士能力评价量表调查临床护士核心能力现状。采用单因素分析及多重线性回归分析进行数据分析。本研究共回收问卷156份,其中有效问卷130份,有效率为83.3%。结果山西省临汾市国家级贫困县医院130名临床护士核心能力总分为(131.20±31.61)分,最低33分,最高212分。单因素分析显示,医院因素中,B医院在评判性思维与科研维度、临床护理维度、教育与咨询维度以及核心能力总分均高于A医院,C医院在评判性思维与科研维度和教育与咨询维度的得分高于A医院,差异均有统计学意义(P<0.05);用工方式因素中,临时工各个维度以及核心能力总分均低于正式在编与合同工,正式在编的护士评判性思维与科研维度、人际关系维度、法律与伦理实践维度的得分以及能力总分均低于合同工,差异有统计学意义(P<0.05);在科室因素方面,儿科病房专业发展维度的得分低于除内科病房以外的其他病房,差异有统计学意义(P<0.05)。多因素回归分析显示,用工方式及科室进入了护士核心能力总分的回归方程,2个影响因素可解释护士核心能力总分总变异的22.5%。结论山西省临汾市国家级贫困县医院临床护士核心能力总体处于中等水平,具备一定的胜任护理工作能力,医院、用工方式及科室对其有不同程度的影响。护理管理者需提高对护士核心能力培养的关注度,更新、完善培养方案,全面提升护士核心能力。
Objective To explore the current status and influencing factors of core competence of clinical nurses in national poverty country hospitals in Linfen,Shanxi Province,so as to provide a basis for nursing managers to establish the nurses training program.Methods From November 1 to November 30,2018,we selected clinical registered nurses in three national poverty country hospitals(A、B、C)in Linfen,Shanxi Province,as subjects by convenience sampling.The self-designed General Information Questionnaire and Competency Inventory for Registered Nurse Scale(CIRN)were used to investigate the core competence of clinical nurses.Data analysis was carried out with the one-way ANOVA and multiple linear regression.A total of 156 questionnaires were collected and 130 of them were valid with 83.3%for the valid rate.Results Among 130 clinical nurses in national poverty country hospitals in Linfen,Shanxi Province,the total score of core competence was(131.20±31.61)with a minimum of 33 and a maximum of 212.Univariate analysis showed that in terms of hospital factors,dimension scores of critical thinking and scientific research,clinical nursing,education and consultation and the total score of core competence of B hospital were higher than those of A hospital;dimension scores of critical thinking and scientific research,education and consultation of C hospital were higher than those of A hospital;those differences were statistical differences(P<0.05).In the respect of employment mode factors,dimension scores and total score of core competence of temporary workers were lower than those of formal recruitment and contract workers;dimension scores of critical thinking and scientific research,interpersonal relationship,law and ethical practice as well as the total score of core competence of formal recruitment nurses were lower than those of contract nurses with statistical differences(P<0.05).In terms of department factors,the score of professional development dimension of Pediatric Ward was lower than those of other wards except for In
