摘要
目的:探讨五味消毒饮对非哺乳期乳腺炎(Non-puerperal Mastitis,NPM)急性期患者的临床疗效,分析影响患者复发的因素。方法:将2017年1月-2018年10月间我院收治的50例非哺乳期乳腺炎急性期患者根据不同的治疗方法分为对照组(口服左氧氟沙星片联合泼尼松龙片)及观察组(口服五味消毒饮),各25例。观察两组治疗后的疗效、复发及不良反应。随访1年,观察两组患者复发的情况并收集其临床资料,用χ2检验分析上述因素对治愈及复发的影响,用多元Cox回归模型筛选患者治愈后复发的影响因素。结果:两组患者的治愈率均为84%(21/25)(P>0.05);两组不良反应(腹泻率,满月脸率,面部痤疮率,体质量增加)比较,P均<0.05;随访1年后,对照组的复发率为38%(8/25),平均复发时间(88.50±19.00)天;观察组的复发率为9.5%(2/25)(P<0.05),平均复发时间(144.00±8.49)d(P<0.05)。χ2检验结果示:乳头内陷(P=0.021)、乳房炎性包块面积(P=0.007)、白细胞数(P=0.027)均可影响患者治疗效果;乳头内陷(P=0.034)、病灶部位(P=0.02)、乳房炎性包块面积(P=0.01)、白细胞数(P=0.03)可影响患者的复发率。多元Cox回归模型分析结果显示,乳腺炎性包块面积比较,OR=1.41,95%CI(1.196~1.662),P<0.05。结论:五味消毒饮治疗非哺乳期乳腺炎急性期患者疗效确切、不良反应少、复发率低。乳房炎性包块面积导致非哺乳期乳腺炎急性期患者复发的危险因素,故选择合适的治疗时机是预防复发的重要措施之一。
Objective:To explore the clinical efficacy of Wuwei Xiaodu Yin(五味消毒饮)in patients with non-lactating mastitis in the acute stage,and analyze the factors that affect the patients’recurrence.Methods:50 cases were divided into the control group(oral levofloxacin tablets plus prednisolone tablets)and observation group(oral Wuwei Xiaodu Yin)according to different treatment methods,25 cases in each.Results:The total efficacy in two groups were 84%(21/25)(P>0.05).The adverse reactions in the control group were more(P<0.05).In 1 year,recurrence in the observation group was fewer(P<0.05).Nipple inversion,breast inflammatory mass area,white blood cell count can affect the treatment effect.Nipple inversion,lesion site,breast inflammatory mass area,white blood cell count can affect the recurrence rate.Multivariate Cox regression model analysis results show that the area of inflammatory mass in breast[OR=1.41,95%CI(1.196~1.662),P﹤0.05]showed differences.Conclusion:Wuwei Xiaodu Yin is effective,with fewer adverse reactions and lower recurrence rate.The area of breast inflammatory mass is the risk factors of recurrence in patients with non-lactating mastitis in the acute stage,so choosing an appropriate treatment timing is one of the important measures to prevent patients from recurrence.
