摘要
目的探讨MRI在结直肠癌术后局部复发治疗方案中的临床价值。方法选取73例于2018年3月至2019年3月入住本院经病理学检查确诊为结直肠癌术后局部复发的患者为研究对象,所有患者均行CT与MRI扫描,比较两种检查方式的诊断准确率与影像学特征表现。结果MRI检查的诊断准确率为94.52%,较CT检查的82.19%明显更高(P<0.05);CT扫描显示患者髋前存在不对称分布的密度均匀或不均匀的软组织肿块,行增强扫描有不同程度的强化反应,肿块边缘有不同程度的环状强化,病灶累及盆腔、膀胱、前列腺、子宫、阴道等;MRI扫描显示,患者骶前部有叶状肿块,吻合口存在不规则软组织肿块,T1WI序列呈低信号,T2WI与增强扫描序列呈高信号,行增强扫描后可见不同程度强化反应,病灶累及膀胱、前列腺、子宫、阴道、精囊腺等,复发肿瘤间未见脂肪间隙,部分患者器官内现类似于肿瘤的信号。结论MRI在结直肠癌术后局部复发治疗方案中具一定临床价值,可有效检测出患者复发情况,且影像学特征表现可反映出病灶的范围及其与周边组织之间的关系,为患者临床治疗方案的制定提供可靠依据。
Objective To explore the clinical value of MRI in the treatment of postoperative local recurrence of colorectal cancer.Methods 73 patients with postoperative local recurrence of colorectal cancer confirmed by pathological examination and treated in our hospital from March,2018 to March,2019 were selected as research objects.All the patients took CT and MRI scanning.The diagnostic accuracies and imaging characteristics were compared between these two methods.Results The diagnostic accuracy of MRI examination was 94.52%,which was significantly higher than that of CT examination(82.19%)(P<0.05).CT scan showed that the patients had asymmetrically distributed soft tissue masses with uniform density or uneven distribution in the front of their hips,and the enhancement response of the patients was different in the enhanced scan;there were different degrees of ring enhancement at the mass edge,and the lesions involved pelvic cavity,bladder,prostate,uterus,vagina,etc.MRI scan showed that the patients got sacral anterior leaf mass,anastomotic irregular soft tissue mass,low signal of sequence T1WI,high signal of T2WI and enhancement scanning sequence,and different degrees of reinforcement reaction after enhancement scanning;the lesions involved bladder,prostate,uterus,vagina,seminal vesicle gland,etc.;no fat gap was between tumor recurrences;and some patients’organs had signal similar to tumor.Conclusion It has a certain clinical value in postoperative local recurrence treatment of colorectal cancer,and can effectively detect the patients’recurrence;and the imaging characteristics can reflect the lesion range and the relationship with peripheral tissue,so as to provide a reliable basis for the formulation of the patients’clinical treatment plan.
