摘要
目的探讨不同麻醉方式对老年骨科患者血流动力学及认知功能的影响。方法选取2017年3月至2019年3月在本院接受手术治疗的90例老年骨科患者,按照随机数字表法分为研究组与对照组,各45例。研究组采用腰麻硬膜外复合麻醉,对照组采用全凭静脉麻醉,比较两组患者血流动力学情况、术后麻醉恢复情况及认知功能。结果与麻醉前比较,麻醉后10、30 min时,两组舒张压(DBP)、收缩压(SBP)及心率(HR)均下降,且研究组下降幅度明显小于对照组(P<0.05);研究组麻醉10、30 min时镇静评分明显低于对照组,麻醉后12、24 h简易精神状况检查量表(MMSE)评分明显高于对照组(P<0.05)。结论腰麻硬膜外复合麻醉用于老年骨科手术,可减轻术中血流动力学应激改变,且术后患者麻醉苏醒、意识恢复效果良好,具有显著应用价值。
Objective To investigate the effects of different anesthesia methods on hemodynamics and cognitive function in elderly orthopedic patients. Methods 90 elderly Orthopedic patients who underwent surgery in our hospital From March 2017 to March 2019 were randomly divided into the study group and the control group, with 45 cases in each group. The study group underwent spinal anesthesia combined with anesthesia, and the control group underwent all intravenous anesthesia. Hemodynamics, postoperative anesthesia recovery and cognitive function between two groups. Results Compared with before anesthesia, the diastolic blood pressure(DBP), systolic blood pressure(SBP) and heart rate(HR) decreased at 10 min and 30 min after anesthesia. The decline of the study group was significantly lower than that of the control group(P<0.05). The sedation score of the study group was significantly lower than that of the control group at 10 min and 30 min after anesthesia. The scores of the mini-mental condition examination scale(MMSE) at 12 h and 24 h after anesthesia were significantly higher than those of the control group(P<0.05). Conclusion Spinal anesthesia combined with epidural anesthesia for elderly orthopedic surgery can reduce the hemodynamic stress changes during operation, and the postoperative anesthesia recovery, consciousness recovery effect is good, has significant application value.
