摘要
目的:了解综合医院康复科护理人员配置和工作内容现况,探讨护士配置数量与康复专科护理实施频次的关系,为规范康复科护理工作内容和合理配置康复科护理人员提供参考依据。方法:通过自行设计的康复科病房基本情况调查表和护理人员工作情况调查表,对浙江省三级综合医院的康复科护理人员进行调查。结果:综合医院康复科床护比为1∶0.32(1∶0.21~1∶0.42)。在护理工作内容的9个维度中,交接班与文书整理实施频次居于首位,康复护理与宣教居于第7位。在康复护理与宣教维度中,护士实施治疗相关宣教频次较高(1~2次/天),对进食、排泄等功能的训练与指导较少(1~2次/周)。责任护士分管患者数<11人时,康复护理与宣教的实施频次随患者数量的减少而增加,β值为-3.4(95%CI:-6.73^-0.03)。结论:不同医院康复科护理人员配置存在较大差异,工作内容现况未能较好体现康复护理专科特色,护士对患者功能的恢复与再建评估和指导较少。责任护士分管患者数影响康复护理实施频次。
Objective:To understand the current status of nursing staffing and work content in the rehabilitation department of general hospitals,to explore the relationship between the number of nurses and the frequency of implementation of specialized rehabilitation care,and to provide a reference basis for regulating the content of nursing work in the rehabilitation department and rational allocation of rehabilitation nursing staff.Methods:A survey of the nursing staff in the rehabilitation department of the tertiary general hospital in Zhejiang Province was conducted through a self-designed questionnaire on the basic conditions of the rehabilitation wards and a questionnaire on the work of the nursing staff.Results:The ratio of bed care in the general hospital rehabilitation department was 1:0.32 (1:0.21 to 1:0.42).Of the nine dimensions of nursing care,handover and documentation are the most frequent,while rehabilitation care and education are in seventh place.In the rehabilitation nursing and education dimension,nurses conducted treatment-related education more frequently (1 to 2 times/day) and provided less training and guidance on eating and excretion (1 to 2 times/week).When the number of patients was less than 11,the frequency of rehabilitation care and education increased with the number of patients,with a β value of-3.4 (95% CI:-6.73 to-0.03).Conclusion:There are significant differences in the nursing staffing of rehabilitation departments in different hospitals,and the current status of work does not better reflect the specialties of rehabilitation nursing.The number of patients assigned by the responsible nurse affects the frequency with which rehabilitation care is implemented.
