摘要
目的探讨在新生儿呼吸机相关性肺炎预防方面应用过氧化氢和碳酸氢钠口腔护理的效果。方法选取我院新生儿科2017年5月至2019年5月收治的接受经口气管插管有创机械通气治疗的120例新生儿为研究对象,随机等分为观察组和对照组。对照组给予常规机械通气护理以及2.5%的碳酸氢钠溶液口腔擦拭;观察组在对照组基础上加1.5%的过氧化氢溶液进行口腔擦拭。比较两组患儿口腔感染率、呼吸机相关性肺炎发生率、机械通气时间。结果观察组口腔感染率、呼吸机相关性肺炎发生率明显低于对照组,且机械通气时间明显短于对照组,差异有统计学意义(P<0.05)。结论过氧化氢联合碳酸氢钠的双重清洁灭菌作用能有效预防机械通气新生儿呼吸机相关性肺炎及口腔感染发生率,缩短机械通气时间,值得临床推广应用。
Objective To explore the effect of oral care by using hydrogen peroxide and sodium bicarbonate in the prevention of neonatal ventilator-associated pneumonia.Methods We selected 120 cases of neonates treated by oral tracheal intubation and invasive mechanical ventilation from May 2017 to May 2019 in our neonatology department as the study subjects,and they were randomly divided into an observation group and a control group.The control group was given routine mechanical ventilation nursing and 2.5%sodium bicarbonate solution for oral wiping,and the observation group added 1.5%hydrogen peroxide solution for oral wiping on the basis of the control group.The rates of oral infection,the incidence rate of ventilator-associated pneumonia,and mechanical ventilation time were compared between the two groups.Results The rate of oral infection and the incidence of ventilator-associated pneumonia in the observation group were significantly lower than those in the control group,and the mechanical ventilation time in the observation group was significantly shorter than that in the control group.The differences were statistically significant(P<0.05).Conclusion The dual cleaning and sterilizing effect of hydrogen peroxide combined with sodium bicarbonate can effectively prevent the incidence rate of ventilator-associated pneumonia and oral infection in neonates with mechanical ventilation,and shorten the time of mechanical ventilation.It was worthy of clinical promotion and application.
