摘要 目的探讨超声引导下动态针尖定位法在桡动脉穿刺置管术中的应用效果。方法选取择期手术术中需行有创动脉血压监测的患者130例,按随机数字表法分为2组:研究组65例采用超声引导下动态针尖定位技术进行穿刺置管;对照组65例采用手指触摸定位法进行穿刺置管。比较2组穿刺成功率(首次穿刺成功率、5 min内穿刺总成功率)、穿刺耗时、穿刺过程疼痛评分以及不良反应情况。结果与对照组比较,研究组穿刺置管首次成功率、5 min内穿刺置管总成功率均显著升高,平均穿刺耗时显著缩短,疼痛VAS评分、不良反应(桡动脉血肿、痉挛)发生率均显著降低,差异均有统计学意义(均P<0.05)。结论超声引导下采用动态针尖定位法能够显著提高桡动脉穿刺置管的成功率,穿刺操作时间短,患者舒适度高,并发症少。 Objective To investigate the efficacy of ultrasound-guided dynamic needle tip positioning technique for radial arterial cannulation.Methods A total of 130 patients requiring intraoperative invasive arterial blood pressure monitoring were randomly assigned to receive either ultrasound-guided dynamic needle tip positioning technique(study group,n=65)or palpation positioning technique(control group,n=65)for radial arterial cannulation.The first-pass success rate,success rate of cannulation within 5 minutes,time consuming for arterial puncture,VAS pain score and puncture-related complications were compared between the two groups.Results Compared with control group,the first-pass success rate and success rate of cannulation within 5 minutes increased and the time consuming for arterial puncture,VAS score and incidence of complications(radial hematoma and spasm)decreased in study group(P<0.05).Conclusion Ultrasound-guided dynamic needle tip positioning technique can increased the success rate of radial artery cannulation,shorten puncture time,enhance comfort and reduced complications.
