摘要
目的:研究变异性心绞痛患者动态心电图(DCG)的特点。方法:选择2017年5月~2018年7月47例变异性心绞痛患者,分析其DCG的表现特点及与冠状动脉造影结果的相关性。结果:47例患者均捕捉到与临床症状相关的一过性ST段抬高,ST段抬高的检出率为100.0%,多呈单向曲线,其中巨型R波型ST段抬高的检出率为23.4%(11/47);T波高耸的检出率为68.1%(32/47);室性期前收缩检出率为55.3%(26/47);房室传导阻滞的检出率为40.4%(19/47);加速性室性逸搏的检出率为34.0%(16/47);U波倒置的检出率为10.6%(5/47)。47例患者变异性心绞痛患者总共发作115次,心绞痛多发生在凌晨(45.2%)与夜间(33.9%),其次为上午(18.3%),下午最少(2.6%)。与冠状动脉造影结果作对照,31例ST段抬高>0.2mV的患者中,其中有27例(87.1%)存在冠状动脉狭窄;20例ST段抬高最长持续时间≥3min的患者中,其中有18例(90.0%)存在冠状动脉狭窄;16例ST段抬高2阵以上的患者中,其中有13例(81.3%)存在冠状动脉狭窄。结论:DCG可显示变异型心绞痛发作的全过程,并且能够提示冠状动脉狭窄的程度。
Objective:To study the characteristics of dynamic electrocardiogram(DCG)in patients with variant angina.Methods:From May 2017 to July 2018,47 patients with variant angina pectoris were selected to analyze the characteristics of DCG and their correlation with coronary angiography results.Results:All 47 patients captured transient ST-segment elevation related to clinical symptoms.The detection rate of ST-segment elevation was 100.0%,and most of them showed a unidirectional curve.Among them,the detection of giant R-wave ST-segment elevation The detection rate was 23.4%(11/47);the detection rate of T wave towering was 68.1%(32/47);the detection rate of pre-ventricular contraction was 55.3%(26/47);atrioventricular block The detection rate was 40.4%(19/47);the detection rate of accelerated ventricular escape beat was 34.0%(16/47);the detection rate of U wave inversion was 10.6%(5/47).47 patients with angina pectoris had 115 episodes in total.Angina mostly occurred in the early morning(45.2%)and night(33.9%),followed by the morning(18.3%)and the least in the afternoon(2.6%).Compared with the results of coronary angiography,of the 31 patients with ST segment elevation>0.2mV,27(87.1%)had coronary artery stenosis;20 patients with the longest duration of ST segment elevation≥3min,among them,18 cases(90.0%)had coronary artery stenosis;of the 16 patients with ST segment elevation of more than 2 arrays,13 cases(81.3%)had coronary artery stenosis.Conclusion:DCG can show the whole process of variant angina pectoris,and can indicate the degree of coronary artery stenosis.
