摘要
目的 分析鉴别变异型心绞痛(VAP)与急性心肌梗死(AMI)中使用心电图的效果。方法 选取2021年2月-2022年3月本院收治的VAP患者中随机抽取50例,均进行心电图和冠状动脉造影检查,分析检测结果。结果 冠状动脉造影检测结果为检测AMI金标准:50例VAP患者中15例发展为AMI。心电图监测结果:17例发展为AMI、误诊2例,检出AMI率组间对比(P>0.05);50例VAP患者的心电图表现:ST段抬高21例,占比42.00%;T波高耸26例,占比52.00%;室性早搏50例,占比100.00%;房室性传导阻滞11例,占比22.00%;U波倒置7例,占比14.00%;R波抬高S波降低6例,占比12.00%。结论 在心电图监测结果方面VAP与AMI患者之间既有一定的联系,又有各自的特异性,而在早期将VAP与AMI鉴别出来,能有效地预防VAP患者转变成为AMI,从而降低AMI发生率。对心电图监测结果进行全面的分析,结合两种疾病的特征,能准确将两种疾病鉴别出来,再结合心肌酶学相关指标检验,能进一步提高诊断结果的准确性。
Objective To analyze effect of electrocardiogram in distinguishing variant angina pectoris(VAP)from acute myocardial infarction(AMI).Methods The paper chose 50 VAP patients admitted to our hospital from February 2021 to March 2022 for electrocardiogram and coronary angiography examinations,and analyzed test results.Results Based on gold standard of coronary angiography for detecting AMI:15 out of 50 VAP patients developed AMI.ECG monitoring results:17 cases developed to AMI and 2 cases were misdiagnosed.Detection rate of AMI compariosn between groups showed(P>0.05).Electrocardiogram manifestations of 50 VAP patients:there were 21 cases of ST segment elevation,accounting for 42.00%;26 cases of T-wave towering,accounting for 52.00%;50 cases of ventricular premature beats,accounting for 100.00%;11 cases of atrioventricular block,accounting for 22.00%;7 cases had U-wave inversion,accounting for 14.00%;6 cases of R-wave elevation and S-wave decrease,accounting for 12.00%.Conclusion There is some certain connection and specificity between VAP and AMI patients in terms of electrocardiogram monitoring results.Distinguishing VAP from AMI at early stage can prevent VAP patients from transitioning to AMI effectively,reduce incidence of AMI.Comprehensive analysis of electrocardiogram monitoring results,combined with characteristics of two diseases,can distinguish two diseases accurately,and further improve accuracy of diagnostic results combined with myocardial enzymology related indicators testing.
