摘要
目的探究解郁清心安神汤联合平腕立指针刺手法治疗抑郁症临床疗效及安全性。方法本次研究共计纳入106例抑郁症患者,纳入患者均由医院2017年3月—2019年1月收治,采取随机数字表法将患者分为观察组和对照组,对照组患者(53例)常规西医治疗,观察组患者(53例)常规西医治疗+解郁清心安神汤联合平腕立指针刺手法治疗,比较两组患者临床治疗效果、治疗前后患者血清单胺类神经递质水平变化、血清炎性因子水平变化、汉密尔顿抑郁量表(HAMD)评分变化及心烦易怒、情绪不宁等症状积分变化、睡眠质量评分(匹兹堡睡眠指数,PSQI)及日常生活能力情况(ADL)、患者生活质量(SF-36)评分、治疗不良反应情况。结果观察组患者经治疗后临床总有效率[96.22%(51/53)]高于对照组[84.91%(45/53)],P<0.05;治疗前,两组患者多巴胺(DA)、去甲肾上腺素(NE)、5-羟色胺(5-HT)等血清单胺类神经递质水平、肿瘤坏死因子-α(TNF-α)、白介素2(IL-2)、白介素6(IL-6)等炎性因子水平均相当,P>0.05,治疗后各组患者DA、NE、5-HT、TNF-α、IL-2、IL-6水平均改善,观察组患者治疗后DA、NE、5-HT、TNF-α、IL-2、IL-6水平优于对照组,P<0.05;治疗前,两组患者HAMD评分、症状总积分、PSQI评分、ADL评分、SF-36评分相当,P>0.05,治疗后各组患者HAMD评分、症状总积分、PSQI评分、ADL评分、SF-36评分均改善,观察组患者治疗后HAMD评分、症状总积分、PSQI评分、ADL评分、SF-36评分优于对照组,P<0.05;观察组患者不良反应[3.77%(2/53)]与对照组[1.89%(1/53)]均较少,不需治疗可自行缓解,P>0.05。结论解郁清心安神汤联合平腕立指针刺手法治疗抑郁症疗效显著,患者症状改善,恢复良好,日常生活能力提升,睡眠质量及生活质量均改善,不良反应少,安全性高,值得推广以及应用。
Objective To explore the clinical efficacy and safety of Jieyu Qingxin Anshen Decoction combined with needling by horizontal wrist and vertical fingers in the treatment of depression.Methods A total of 106 depression patients in our hospital from March 2017 to January 2019 were analyzed.By taking a random number table method,the patients were dividedinto observation group and control group.The control group(53 cases)was treated with routine western medicine treatment and the observation group(53 cases)was treated with routine western medicine treatment+Jieyu Qingxin Anshen Decoction combined withneedling by horizontal wrist and vertical fingers.We compared the clinical treatment effect,patients’blood listing amine neurotransmitter level changes,changes in serum levels of inflammatory cytokines,Hamilton depression scale(HAMD)scores and upset symptoms such as irritability,mood restless integral change and sleep quality score(Pittsburgh sleep index,PSQI)and ability of daily life(ADL),patients’quality of life(SF-36)score and the adverse reactions.Results The total clinical effective rate[96.22%(51/53)]in the observation group was higher than that in the control group[84.91%(45/53)](P<0.05).Beforetreatment,two groups’dopamine(DA),norepinephrine(NE)and serotonin(5-HT)and other blood listing amine neurotransmitter levels,tumor necrosis factor alpha(TNF-α),interleukin 2(IL-2)and interleukin 6(IL-6),and other inflammatory factor levels were fair(P>0.05).After treatment,levels of DA,NE,5-HT,TNF-α,IL-2 and IL-6 wereincreased.The observation group’s levels of DA,NE,5-HT,TNF-alpha,IL-2 and IL-6 were better than those of the control group(P<0.05).HAMD scores,symptoms integral,PSQI scores and ADL scores and SF-36 scalebefore treatment of the two groups were not significant(P>0.05).After treatment,each group’s HAMD scores symptom integral,PSQI scores,ADL scoresand SF-36 score were improved,observation group after treatment in patients with HAMD scores,symptom integral.PSQI scores,ADL scores and SF-36 score were bette
