摘要 目的:分析程序性死亡分子配体1(PD-L1)在宫颈癌患者中的表达调控机制和对人乳头瘤病毒(HPV)感染的预后评价效果。方法:选取2011年1月-2018年3月首次治疗的HPV感染伴宫颈脱落细胞学检查患者108例,其中HPV感染宫颈细胞未病变54例为观察组1,HPV感染伴有宫颈病变54例为观察组2。另选取未感染HPV的患者40例为对照组。检测三组宫颈脱落细胞表面PD-L1表达情况。结果:观察组2 PD-L1阳性细胞比例和阳性细胞平均荧光道数均明显高于其他两组,差异有统计学意义(P<0.05);观察组1和对照组PD-L1阳性细胞比例和阳性细胞平均荧光道数比较,差异无统计学意义(P>0.05)。结论:宫颈癌患者HPV宿主细胞基因组整合状态均能够上调肿瘤细胞表面的PD-L1表达,有利于抗体药物在宫颈癌患者中的应用,宫颈细胞表面的PD-L1表达水平也可作为HPV感染预后清除有效的指标。 Objective:To explore the mechanism of PD-L1 expression regulation in cervical cancer patients and its prognostic value for HPV infection.Methods:From January 2011 to March 2018,108 patients with the first treatment for HPV infection and cervical exfoliative cytology were selected,among them,54 patients with HPV infection but without cervical cytopathy were selected as the observation group 1,and 54 patients with HPV infection and cervical lesions were selected as the observation group 2.Another 40 patients without HPV infection were selected as the control group.We detected the expression of PD-L1 on the surface of cervical exfoliated cells in three groups.Results:In the observation group 2,the proportion of PD-L1 positive cells and the average number of fluorescent channels of positive cells were significantly higher than those of the other two groups,the difference was statistically significant(P<0.05).There was no significant difference in PD-L1 positive cells and the average number of fluorescent channels of positive cells between the observation group 1 and control group(P>0.05).Conclusion:The integration of HPV host cell genome in cervical cancer patients can up regulate the expression of PD-L1 on the surface of tumor cells,it is beneficial to the application of antibody drugs in cervical cancer patients,the expression level of PD-L1 on the surface of cervical cells can also be used as an effective indicator of the prognosis of HPV infection.
