富集自体骨髓间质干细胞在兔椎间关节融合中的应用研究

摘要 目的探讨富集自体骨髓间质干细胞在兔椎间关节融合中的应用效果。方法选取健康10周龄的新西兰大白兔96只,雌雄不限,随机取新西兰大白兔24只,设为空白对照组(4组)。剩余72只新西兰大白兔均经前路腹膜外行腰椎间盘切除脊柱融合术,建立新西兰大白兔动物模型。建模成功后随机分为1组(n=24)、2组(n=24)、3组(n=24)。1组建模成功后不采取任何措施处理,常规饮食、饮水;2组给予纤维蛋白胶、骨形态发生蛋白干预;3组在2组基础上联合富集自体骨髓间质干细胞移植干预,比较四组大白兔第12周时阶段活动程度、四组大白兔椎体后缘高度丢失情况、椎间关节融合情况、X线侧位片上测量Cobb角情况。结果1组关节活动范围与活动度低于2组、3组、4组(P<0.05),2组、3组关节活动范围与活动度相比,无明显差异(P>0.05),4组关节活动范围与活动度最差;四组大白兔椎体后缘高度丢失情况相比,无明显差异(P>0.05);1组椎间关节融合分级高于2组、3组、4组(P<0.05),2组、3组关节融合分级相比,无明显差异(P>0.05),4组关节融合分级最差;四组大白兔Cobb角相比,无明显差异(P>0.05)。结论兔椎间关节融合中,富集自体骨髓间质干细胞移植效果显著,可以有效促进兔椎间关节融合,应在临床上广泛推广与应用。 Objective To explore the application effect of enriched autologous bone marrow mesenchymal stem cells in the rabbit intervertebral joint fusion.Methods Ninety-six healthy New Zealand rabbits aged 10 weeks were selected of either sex.Twenty-four New Zealand rabbits were randomly selected as the blank control group(group 4).The remaining 72 New Zealand rabbits were given extraperitoneal lumbar discectomy and spinal fusion via the anterior approach,and the animal model of New Zealand rabbits was established.Those rabbits were randomly divided into group 1(n=24),group 2(n=24)and group 3(n=24)after modeling.No intervention was given to group 1 after modeling,which was fed with regular diet and drinking water;the intervention of fibrin glue and bone morphogenetic protein was given to group 2;the intervention of enriched autologous bone marrow mesenchymal stem cells was given to group 3 on the basis of group 2.At 12 weeks,the activity level,the loss of height of vertebral posterior margin,the intervertebral joint fusion,and the measured Cobb angle on the lateral X-ray were compared in the four groups.Results The range of joint activity and the activity level in the group 1 were lower than those in group 2,group 3 and group 4(P<0.05);there was no significant difference in the range of joint activity or the activity level between group 2 and group 3(P>0.05);the range of joint activity and the activity level in group 4 were lower than those in the other three groups.There was no significant difference in the loss of height of vertebral posterior margin among the four groups(P>0.05).The grade of the intervertebral joint fusion in the group 1 were higher than those in group 2,group 3 and group 4(P<0.05);There was no significant difference in the grade of the intervertebral joint fusion between group 2 and group 3(P>0.05);The grade of the intervertebral joint fusion in group 4 was lower than that in the other three groups.There was no significant difference in the Cobb angle among the four groups(P>0.05).Conclusion In the rab
