摘要
目的分析结核菌/艾滋病病毒(TB/HIV)双重感染者抗结核治疗成功率,探讨影响治疗成功(治愈+完成疗程)的因素。方法收集2010-2015年结核病管理信息系统和TB/HIV双重感染年度报表数据,用SPSS17.0统计软件,进行单因素和多因素Logistic回归分析。结果 210例TB/HIV双重感染患者抗结核治疗成功率为89.52%。单因素分析结果显示,主动就诊患者的治疗成功率(95.70%)与转诊或追踪患者的治疗成功率(84.62%)之间差异有统计学意义,比值比(OR)值为4.045,95%可信区间(CI)值为1.319~2.406(P <0.05)。多因素分析结果显示,患者来源(OR=5.304;95%CI:1.630~17.261;P <0.05)、登记分类(OR=2.914;95%CI:1.181~7.191;P <0.05)、结核类型(OR=3.362;95%CI:1.060~10.663;P <0.05)、诊断延误天数(OR=0.343;95%CI:0.140~0.838;P <0.05)等因素与患者抗结核治疗成功有关。结论加强宣传教育,提高结核病防治知识知晓率,促进患者及时主动就诊,有利于提高抗结核治疗成功率。
Objective To analyze the success rate of anti-tuberculosis treatment in TB/HIV co-infection patients, and to explore factors affecting the success of the treatment. Methods We collected the data from the TB management information system and the annual report of TB/HIV co-infection during 2010-2015. Data were analyzed by using SPSS 17.0, and logistic regression was used for univariate and multivariate statistics. Results The success rate of anti-tuberculosis treatment in TB/HIV co-infected patients was 89.52%. Univariate analysis showed that the success rate of active treatment was 95.70% and referral or tracing was 84.62%, with the difference statistically significant(OR= 4.045, 95%CI= 1.319-12.406 P < 0.05). Multivariate analysis indicated that the factors were associated with the success of anti-tuberculosis treatment including source of patients(OR =5.304;95%CI =1.630-17.261;P < 0.05), classification of registration(OR =2.914;95%CI =1.181-7.191;P < 0.05), types of tuberculosis(OR =3.362;95%CI =1.060-10.663;P < 0.05), and days of diagnosis delay(OR =0.343;95%CI =0.140-0.838;P < 0.05). Conclusion Improvement of success rate of anti-tuberculosis treatment needs to strengthen health publicity and education, improve the knowledge awareness of TB prevention and control, and promote patients for active treatment.
