摘要
目的:分析肺结核合并糖尿病患者的临床特征。方法:将80例肺结核合并糖尿病的患者作为观察组,80例肺结核未合并糖尿病的患者设为对照组。观察和分析2组患者流行病状态、临床特征及临床预后情况。结果:观察组在年龄、是否本地、是否初诊、症状、痰涂片结果、肺外结核方面与对照组比较,差异具有统计学意义(P<0.05);2组在性别、职业、肺野受累方面比较,差异无统计学意义(P>0.05)。可以认为年龄、是否本地、痰涂片结果、肺外结核、血糖控制情况、结核类型是肺结核合并糖尿病患者预后的独立影响因素。观察组治愈率、治疗中断率均低于对照组,但组间比较,差异无统计学意义(P>0.05);而无效或病死率高于对照组,组间比较,差异具有统计学意义(P<0.05)。结论:肺结核合并糖尿病患者具有其独特的临床特征,治疗无效或病死率也较高,应加强预后影响因素的干预。
Objective: To investigate the clinical features of patients with pulmonary tuberculosis and diabetes.Methods: A total of 80 patients with pulmonary tuberculosis and diabetes were enrolled as observation group,and 80 pulmonary tuberculosis patients without diabetes were enrolled as control group.The two groups were analyzed in terms of the conditions of the epidemic disease,clinical features,and prognosis.Results: There were significant differences between the two groups in age,local or non-local residency,presence or absence of previous treatment,symptoms,sputum smear results,and extrapulmonary tuberculosis(P>0.05),while there were no significant differences in sex,occupation,and lung involvement between the two groups(P<0.05).Age,local or non-local residency,sputum smear results,extrapulmonary tuberculosis,blood glucose control,and type of tuberculosis were considered independent risk factors for the prognosis of patients with pulmonary tuberculosis and diabetes.Compared with the control group,the observation group had lower cure rate and rate of treatment discontinuation(P>0.05) and significantly higher non-response rate and mortality rate(P <0.05).Conclusion: Patients with pulmonary tuberculosis and diabetes have unique clinical features with high non-response rate and mortality rate,and the intervention for prognostic factors should be enhanced.
