摘要
目的探讨胺碘酮治疗老年慢性心力衰竭(CHF)合并心律失常的效果。方法选取东莞市中医院2017年8月至2019年8月收治的70例老年CHF合并心律失常患者随机分为常规组和治疗组,每组35例。2组均给予常规抗心衰药物治疗,治疗组加用胺碘酮。比较2组患者治疗效果;心律失常转为窦性心律所需时间、短阵室性心动过速发作减少50%的时间、频发室性期前收缩减少50%的时间;治疗前后心功能状态QTC、射血分数、纽约心功能分级及心室率。结果治疗组总有效率高于常规组(100.00%vs.74.29%,P<0.05)。治疗后治疗组心功能状态QTC、射血分数、纽约心功能分级、心室率优于常规组(P<0.05)。治疗组心律失常转为窦性心律所需时间、短阵室性心动过速发作减少50%的时间、频发室性期前收缩减少50%的时间短于常规组(P<0.05)。结论老年CHF合并心律失常患者给予常规抗心衰药物治疗联合胺碘酮辅助治疗效果确切,可有效改善患者心功能,控制心律失常,缩短治疗时间。
Objective To study the effect of amiodarone in the treatment of senile chronic heart failure(CHF)combined with arrhythmia.Methods Totally 70 elderly CHF patients combined with arrhythmia admitted to Dongguan Traditional Chinese Medicine Hospital from August 2017 to August 2019 were randomLy divided into 2 groups,35 cases per group.Both groups were treated with conventional anti-heart failure drugs.The treatment group was treated with amiodarone in addition.The therapeutic effect,required time for arrhythmia converting into sinus rhythm,the time of attack of short-array ventricular tachycardia reduced by 50%,and the time of attack of frequent ventricular premature contraction reduced by 50%,as well as cardiac functional state(QTC),ejection fraction,NYHA grades and ventricular rate before and after treatment were compared between the two groups.Results The total effective rate of the treatment group was higher than that in the conventional group(100.00% vs.74.29%,P<0.05).The cardiac functional state(QTC),ejection fraction,NYHA grades and ventricular rate after treatment in the treatment group were superior to those in the conventional group.The required time for arrhythmia converting into sinus rhythm,the time of attack of short-array ventricular tachycardia reduced by 50%,and the time of attack of frequent ventricular premature contraction reduced by 50% in the treatment group was shorter than the conventional group(P<0.05).Conclusion The effect of routine anti-heart failure drugs combined with amiodarone was effective on elderly CHF patients complicated with arrhythmia.It can effectively improve cardiac function of patients,control arrhythmia and shorten the treatment time.
