摘要
目的探讨胺碘酮治疗慢性心衰合并心律失常对患者心功能的影响,并对安全性进行分析。方法以我院收治的82例慢性心衰合并心律失常患者为研究对象,随机分为观察组(41例,给予胺碘酮+常规治疗)和对照组(41例,给予常规治疗),治疗4周后,观察两组患者治疗前后血压、心率、心电图QT离散度(QTc)变化情况,根据患者心功能及心律改善情况评价疗效,并比较两组用药安全性。结果观察组心功能及心律总有效率显著高于对照组(80.49%vs 65.85%;87.81%vs 75.61%),血压、心率及QTc改善情况明显优于对照组,差异均具有统计学意义(P<0.05);观察组不良反应发生情况与对照组比较差异无统计学意义(P>0.05)。结论胺碘酮治疗慢性心衰合并心律失常疗效显著,能明显改善患者心功能及心律,且安全性较高,值得临床医师参考。
Objective To investigate the effect of amiodarone on cardiac function and its safety in the treatment of patients with chronic heart failure complicated by arrhythmia. Methods A total of 82 patients with chronic heart failure complicated by arrhythmia who were admitted to our hospital were enrolled and randomly divided into observation group （41 patients treated with amiodarone and conventional treatment） and control group （41 patients given conventional treatment alone）. After 4 weeks of treatment, the changes in blood pressure, heart rate, and corrected QT dispersion （QTc） dispersion on electrocardiogram were observed, and the outcome was evaluated based on the improvements in cardiac function and cardiac rhythm; the two groups were also compared in terms of safety. Results Compared with the control group, the observation group had significantly higher overall response rates of cardiac function （80.49% vs 65.85%） and cardiac rhythm （87.81% vs 75.61%） and significantly greater improvements in blood pressure, heart rate, and QTc （all P〈 0.05）. There was no significant difference in the incidence of adverse reactions between the two groups （P〉0.05）. Conclusion Amiodarone has a marked clinical effect in the treatment of chronic heart failure complicated by arrhythmia and can significantly improve patient＇s cardiac function and cardiac rhythm with good safety. Therefore, it holds promise for clinical application.
