摘要
消化系统恶性肿瘤的发病率高,发病隐匿,诊断和治疗方法有限,严重威胁着人类的健康,大部分患者发现时已错失最佳治疗时机。目前消化系统肿瘤的治疗仍以手术及放化疗为主,但生存率并没有明显的改变。近几年,以程序性死亡受体-1(PD-1)及程序性死亡配体-1(PD-L1)为靶点的免疫治疗已经应用于恶性黑色素瘤、非小细胞肺癌等,在胃癌中的应用也处于临床试验阶段,但仍存在一定的局限性。本文将对PD-1/PD-L1的结构功能、PD-1/PD-L1在消化系统恶性肿瘤中的表达及研究、PD-1/PD-L1的病理判读及相关免疫抑制剂的应用进行讨论,旨在为消化系统肿瘤的诊断和治疗提供理论依据。
The high incidence of digestive system tumors,with hidden occurrence and limited diagnosis and treatment methods,threatens human health seriously.Most patients have missed the best opportunity for treatment.Surgery,radiotherapy and chemotherapy are still the main treatment options,for patients with digestive system tumors,but the survival rate has not changed significantly.In recent years,immunotherapies targeting PD-1 and PD-L1 have been applied to treat malignant melanoma and non-small cell lung cancer.But its application in treating gastric cancer in the clinical trial stage is limited.This article has discussed the structure and function of PD-1/PD-L1,the expression of PD-1/PD-L1 in malignant tumors of the digestive system,the pathological interpretation of PD-1/PD-L1,and the application of related immunity inhibitors,which aims to provide a theoretical basis for the diagnosis and treatment of digestive system tumors.
