子宫内膜息肉复发患者孕酮抵抗发生机制的临床研究 被引量:4

摘要 目的通过检测子宫内膜息肉初发组/复发患者子宫内膜息肉组织和对照组正常子宫内膜组织中孕酮受体(progesterone receptor,PR)、雌激素受体(estrogen receptor,ER)和甾体激素受体辅激活子-2(steroid receptor coactivator-2,SRC-2)的表达,探讨子宫内膜息肉初发组及复发组患者是否存在孕酮抵抗现象,以SRC-2在孕酮抵抗中发挥作用机制。方法随机选取2017年1月-2019年1月在我院行宫腔镜下子宫内膜息肉电切或者美奥舒子宫内膜息肉切除术的子宫内膜息肉初发组45例和复发组30例和正常内膜对照组40例,观察ER、PR和SRC-2在3组的表达情况,并进行比较。结果PR和SRC-2在子宫内膜息肉初发组及复发组中的表达显著低于对照组(P<0.05),而ER表达在子宫内膜息肉初发组,复发组和正常内膜组织对照组之间无差异(P>0.05);PR表达与SRC-2表达的偏相关系数=0.7929(P=0.00),相关密切,两者的简单相关系数=0.8107(P=0.00)。结论子宫内膜息肉初发组和复发组均存在孕酮抵抗现象,息肉组织中存在PR、SRC-2低表达,而SRC-2是子宫内膜PR的优势辅激活子,故而降低了子宫内膜PR对孕酮的反应性。这可能就是子宫内膜息肉发生及复发者出现孕酮抵抗现象的分子机制之一。 Objective Detection of progesterone receptor(PR),estrogen receptor(ER)and steroid receptor coactivator-2(SRC-2)in endometrial polyps in primary/relapsed patients was used to investigate whether progesterone resistance exists in patients with primary and relapsed endometrial polyps and whether SRC-2 plays a role in progesterone resistance.Methods A random selection of endometrial polyps who underwent hysteroscopic resection of endometrial polyps or Myosure at Jiangxi Maternal and Child Health Hospital from January 2017 to January 2019.45 cases in primary and 30 cases of in primary and 40 cases of control group.The expressions of ER,PR and SRC-2 in the endometrial polyp in the primary group,and in the relapse group and normal endometrial tissue were observed and compared.Results The expressions of PR and SRC-2 in the endometrial polyp of primary and relapse group were significantly lower than those in the control group(P<0.05),while the expression of ER was higher(P>0.05).The partial correlation coefficient between PR expression and SRC-2 expression was 0.7929(P=0.00),and the correlation was close.The simple correlation coefficient between the two was 0.8107(P=0.00).Conclusion Progesterone resistance was found in both the onset group and the relapse group with the low expresseion of PR,RC-2.SRC-2 is the predominant co-activator of endometrial PR,which reduces the response of endometrial PR to progesterone.This may be one of the molecular mechanisms of progesterone resistance in endometrial polyps.
