早期乳腺浸润性导管癌组织中雌激素受体、孕激素受体、细胞核增殖相关抗原Ki-67的表达情况及临床意义 被引量:4

摘要 目的探讨早期乳腺浸润性导管癌组织中雌激素受体(ER)、孕激素受体(PR)、细胞核增殖相关抗原Ki-67的表达情况及临床意义。方法选择102例早期乳腺浸润性导管癌患者,检测乳腺浸润性导管癌组织中ER、PR、Ki-67的表达情况,分析其与乳腺癌患者临床特征及预后的关系,采用Spearman相关分析法分析ER、PR、Ki-67表达情况的相关性。结果102例患者中,ER、PR和Ki-67的阳性表达率分别为59.80%、56.86%和68.63%。不同年龄、肿瘤直径、临床分期的早期乳腺浸润性导管癌患者中ER、PR的阳性表达率比较,差异均无统计学意义(P﹥0.05)。不同年龄的早期乳腺浸润性导管癌患者中Ki-67的阳性表达率比较,差异无统计学意义(P﹥0.05);肿瘤直径≥2 cm、TNM分期为Ⅱ期的早期乳腺浸润性导管癌患者中Ki-67的阳性表达率分别高于肿瘤直径﹤2 cm、TNM分期为Ⅰ期的患者,差异均有统计学意义(P﹤0.05)。ER与PR表达呈正相关(r=0.425,P﹤0.05),ER与Ki-67表达呈负相关(r=-0.217,P﹤0.05),PR与Ki-67表达呈负相关(r=-0.264,P﹤0.05)。Ki-67阴性患者的5年生存情况优于Ki-67阳性患者,差异有统计学意义(P﹤0.05)。结论 ER、PR及Ki-67在乳腺浸润性导管癌的发生及发展过程中具有重要意义,能够作为预测乳腺癌生物学行为和预后的重要指标。 Objective To investigate the expression and clinical significance of estrogen receptor(ER),progesterone receptor(PR)and proliferation associated nuclear antigen Ki-67 in early breast invasive ductal carcinoma(IDC)tissues.Method The clinical data of 102 cases with early breast IDC were analyzed,the expression of ER,PR and Ki-67 were determined in these tissues collected from corresponding patients,so as to investigate the relationship between their expression and clinical characteristics in breast IDC patients.Spearman correlation analysis was adopted to assess the correlation among ER,PR and Ki-67 expression.Result The positive expression rates of ER,PR and Ki-67 in 102 patients were 59.80%,56.86%and 68.63%,respectively.The positive expression rates of ER and PR were similar in patients with early breast IDC with different age,tumor size and clinical staging(P>0.05).Analogous relationship was observed for positive Ki-67 expression and patients’differing age(P>0.05);tumor diameter≥2 cm and TNM stage II were associated with higher positive expression of Ki-67 as compared to tumor diameter<2 cm and TNM stage I in patients with early breast IDC,the difference was of statistical significance(P<0.05).There was a positive correlation between ER and PR expression(r=0.425,P<0.05),and a negative correlation between ER and Ki-67 expression(r=-0.217,P<0.05),besides,PR was also inversely correlated with Ki-67 expression(r=-0.264,P<0.05).The 5-year survival of patients with negative Ki-67 expression was improved compared to those with positive Ki-67 expression(P<0.05).Conclusion ER,PR and Ki-67 are essentially involved with the occurrence and development of breast IDC,and thus may be of key predictive value for the biological behavior and prognosis of breast cancer.
