摘要
目前我国大力开展分级诊疗制度,全科医师作为居民健康守门人面临严重短缺。因此全科住院医师规范化培养具有重要意义。目前我国全科医学规范化培训体系还不够完善,学生普遍反映社区实习未能达到理想效果;医院轮转时不能从全科医学角度分析和讲解,缺乏全科特色。我院作为全科医学住培基地,在全科医学住院医师培训带教方面进行了探索,包括实行多导师制,加强学员考核,进行师资培训和管理,应用PBL教学法,轮转全科门诊,在考核中应用Mini-cex及SOAP病例汇报,最后积极听取学员反馈,将反馈作为师资考核的指标等。通过以上探索学员培训质量大有提高,但是仍需要加强学员医学人文方面的培训,希望能够实现多学科病例讨论式教学,并着手建立标准化全科疾病病例库,丰富全科医学的带教方式,提高学员学习兴趣。
At present,China has vigorously carried out the hierarchical medical system.General practitioners as residents'health gatekeepers are facing a serious shortage.For the purpose of improving the level of community medical and health services,establishing hierarchical medical system,deepening medical and health system reform and enhance the health of the people,it is of great significance to standardize the training of general practitioners.As for the current situation,the standardized training system of general practitioners in our country is not perfect enough and the training mode needs to be improved.By learning the mature training mode of foreign countries in general practice,we can learn from experience and improve the general residential training mode in China.The Shengjing Hospital of China Medical University,as the general medical residence training base,has explored the teaching methods and contents suitable for the training of general practitioners.It mainly includes:perfecting the management system of general practitioners,training before entering the hospital and implementing a multi tutor system.We carried out the training and management of the students the teachers,applied the PBL teaching method,arranged the rotating general clinic,applied the case reports on Mini-cex and SOAP pathography in the examination,arranged the lectures and special training for different aspects of the general medical characteristics,and finally listened to the feedback from the students,and put the feedback as an evaluation index for the teachers and so on.Through the above exploration, the quality of training has been greatly improved,at the same time training for medical humanities is still needed. We hopeto achieve multidisciplinary case discussion teaching, embark on theestablishment of standardized case database for general practice, enrichthe teaching methods of general practice, and improve students' interest inlearning.
