摘要
目的探讨超声弹性成像与超声造影在乳腺癌诊断与预测新辅助化疗效果中的应用价值。方法选择2016年6月—2019年6月收治的112例初治乳腺肿块作为研究对象,所有良性肿瘤患者治疗前及恶性肿瘤患者行新辅助化疗前后均接受超声弹性成像与超声造影检查,并以病理检查结果为金标准,分析超声弹性成像、超声造影与病理检查结果诊断乳腺癌及评价乳腺癌新辅助化疗效果的一致性。结果本研究共检出肿块124个。病理检查结果显示,恶性肿块70个,良性肿块54个;超声弹性成像检查结果显示,恶性肿块、良性肿块各62个;超声造影检查结果显示,恶性肿块65个,良性肿块59个。超声弹性成像与超声造影诊断乳腺癌的灵敏度分别为81.43%、88.57%,特异度分别为90.74%、94.44%,准确率分别为85.48%、91.13%,阳性预测值分别为91.94%、95.38%,阴性预测值分别为79.03%、86.44%,Kappa值分别为0.71、0.82。本研究乳腺癌62例。病理检查结果显示,乳腺癌新辅助化疗有效49例,无效13例;超声弹性成像结果显示,乳腺癌新辅助化疗有效46例,无效16例;超声造影检查结果显示,乳腺癌新辅助化疗有效47例,无效15例。超声弹性成像与超声造影评价乳腺癌新辅助化疗效果的灵敏度分别为85.71%、89.80%,特异度分别为69.23%、76.92%,准确率分别为82.26%、87.10%,阳性预测值分别为91.30%、93.62%,阴性预测值分别为56.25%、66.67%,Kappa值分别为0.51、0.63。结论超声弹性成像与超声造影在乳腺癌诊断与预测新辅助化疗效果中均具有重要的应用价值。
Objective To study the application value of ultrasound elastography and contrast-enhanced ultrasound in the diagnosis and prediction of breast cancer in patients undergoing neoadjuvant chemotherapy.Methods A total of 112 cases of newly treated breast lump from June 2016 to June 2019 were selected as the research subject.All patients with benign tumors and patients with malignant tumors underwent ultrasound elastography and contrast-enhanced ultrasound before and after neoadjuvant chemotherapy,and the results of pathological examination served as the gold standard.The consistent results of ultrasound elastography,contrast-enhanced ultrasound and pathological examination were compared in diagnosis of breast cancer and evaluation of the efficacy of neoadjuvant chemotherapy for breast cancer.Results A total of 124 lumps were detected in this study.The results of pathological examination showed 70 malignant lumps and 54 benign ones.Ultrasound elastography revealed 62 malignant and 62 benign lumps.Contrast-enhanced ultrasound revealed 65 malignant and 59 benign ones.The sensitivity,specificity,accuracy,positive predictive value,negative predictive value,and kappa value were 81.43%vs.88.57%,90.74%vs.94.44%,85.48%vs.91.13%,91.94%vs.95.38%,79.03%vs.86.44%,and 0.71 vs.0.82,respectively.Sixty-two cases of breast cancer were reported in this study.Pathological examination showed that neoadjuvant chemotherapy for breast cancer was effective in 49 cases and ineffective in 13 cases.Ultrasound elastography showed that neoadjuvant chemotherapy for breast cancer was effective in 46 cases and ineffective in 16 cases,and ultrasound contrast examination showed that neoadjuvant chemotherapy for breast cancer was effective in 47 cases and ineffective in 15 cases.The sensitivity,specificity,accuracy,positive predictive value,negative predictive value and kappa value of neoadjuvant chemotherapy for breast cancer were 85.71%vs.89.80%,69.23%vs.76.92%,82.26%vs.87.10%,91.30%vs.93.62%,56.25%vs.66.67%and 0.51 vs.0.63,respectively.Conclusion Ul
