摘要
目的研究超声造影(CEUS)用于乳腺癌预后判断的临床价值,以期更好地预测乳腺癌患者预后情况。方法选取2019年1月至2020年6月万宁市人民医院收治的50例乳腺癌患者为研究对象进行回顾性分析,根据不同预后将其分为预后不良组(8例,死亡)和预后良好组(42例,存活),记录两组患者CEUS临床特征。采用多因素Logistic分析法分析乳腺癌预后相关因素,分析CEUS相关指标判断预后的价值。结果50例患者中死亡8例,死亡率16.00%。多因素Logistic分析显示,肿瘤分期(95%CI=1.114~4.153,OR=2.151,P=0.023)、分化程度(95%CI=1.278~8.336,OR=3.264,P=0.013)、达峰时间(95%CI=0.549~0.947,OR=0.721,P=0.019)及峰值强度(95%CI=1.223~2.197,OR=1.639,P=0.001)与乳腺癌预后密切相关。根据多因素Logistic分析结果建立预测模型:Y_(1)=0.776X_(1)+1.183X_(2),Y 2=0.776X_(1)+1.183X_(2)-0.327X_(6)+1.021X_(7)。受试者工作曲线(ROC)分析显示,模型Y 2判断预后的曲线下面积(AUC)值为0.833,显著高于模型Y_(1)的AUC值(P<0.05)。结论CEUS定量指标与患者预后密切相关,以此建立预测模型有助于提高判断预后的准确性。
Objective To study the clinical value of contrast-enhanced ultrasound(CEUS)in predicting the prognosis of breast cancer,in order to better predict the prognosis of breast cancer patients.Methods 50 patients with breast cancer admitted to People’s Hospital of Wanning from January 2019 to June 2020 were selected as the research subjects,and the patients were divided into unfavourable prognosis group(8 cases,death)and favourable prognosis group(42 cases,survival),which accorded to different prognosis.The clinical characteristics of CEUS in the two groups were recorded.The Logistic multivariate analysis was used to analyze the prognostic factors of breast cancer,and the prognostic value of CEUS related indexes were analyzed.Results There were 8 cases of the 50 patients died,therefore the mortality rate was 16.00%.The Logistic multivariate analysis showed that tumor stage(95%CI=1.114~4.153,OR=2.151,P=0.023),differentiation degree(95%CI=1.278~8.336,OR=3.264,P=0.013),peak time(95%CI=0.549~0.947,OR=0.721,P=0.019)and peak intensity(95%CI=1.223~2.197,OR=1.639,P=0.001)were closely related to the prognosis of breast cancer.The prediction model was established according to the results of multiple Logistic analysis:Y_(1)=0.776X_(1)+1.183X_(2),Y 2=0.776X_(1)+1.183X_(2)-0.327X_(6)+1.021X_(7).The receiver operating characteristic(ROC)analysis showed that the area under the curve(AUC)of model Y 2 to judge the prognosis was 0.833,which was significantly higher than that of model Y_(1) of AUC(P<0.05).Conclusion CEUS quantitative indicators are closely related to the prognosis of patients,so the establishment of a prediction model is helpful to improve the accuracy of prognosis.
