摘要
目的监测多囊卵巢综合征(PCOS)患者孕中期血清心肌酶水平及左心室功能的变化,分析PCOS患者孕期心功能的变化及意义。方法选取2018年1—11月华北理工大学附属医院的PCOS孕妇98例为观察组,正常孕妇107例为对照组。所有研究对象均在孕前及孕中期(31~32周)检验血清肌酸激酶、肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(α-HBDH)水平,并行超声检查测定左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)、左心房内径(LAD)。对2组孕妇的检测结果进行统计分析。结果2组孕妇孕前心肌酶水平及左心室功能相关指标差异均无统计学意义(均P>0.05)。孕中期观察组肌酸激酶、CK-MB、LDH、α-HBDH水平均明显高于对照组[(356±46)U/L比(310±43)U/L、(97±47)U/L比(80±31)U/L、(523±22)U/L比(472±13)U/L、(174±24)U/L比(150±20)U/L],LVEDD、LVESD、LAD明显大于对照组[(49.4±3.6)mm比(42.2±3.2)mm、(35.3±7.2)mm比(32.6±6.5)mm、(36±13)mm比(33±14)mm],LVEF明显低于对照组[(48±9)%比(53±10)%],差异均有统计学意义(均P<0.05)。结论PCOS孕妇发生心功能不良的风险较高,应在孕中期监测血清心肌酶水平及心脏超声检查以评估心脏功能。
Objective To analyze the changes of serum myocardial enzymes and left ventricular function during the second trimester of pregnancy in patients with polycystic ovarian syndrom(PCOS).Methods From January to November 2018,98 pregnant women with PCOS and 107 healthy pregnant women admitted to the Affiliated Hospital of North China University of Science and Technology were enrolled.Serum creatine kinase,creatine kinase isoenzyme(CK-MB),lactate dehydrogenase(LDH)andα-hydroxybutyric dehydrogenase(α-HBDH)were detected,echocardiographic parameters including left ventricular end diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF)and left atrial diameter(LAD)were measured before pregnancy and during the second trimester of pregnancy.Results There were no significant differences in serum myocardial enzymes and left ventricular function indexes between PCOS and normal pregnant women before pregnancy(all P>0.05).During the second trimester of pregnancy,levels of serum creatine kinase,CK-MB,LDH andα-HBDH in PCOS women were higher than those in normal women[(356±46)U/L vs(310±43)U/L,(97±47)U/L vs(80±31)U/L,(523±22)U/L vs(472±13)U/L,(174±24)U/L vs(150±20)U/L];LVEDD,LVESD and LAD in PCOS women were higher and LVEF was lower than those in normal women[(49.4±3.6)mm vs(42.2±3.2)mm,(35.3±7.2)mm vs(32.6±6.5)mm,(36±13)mm vs(33±14)mm,(48±9)%vs(53±10)%],the differences were statistically significant(all P<0.05).Conclusions PCOS women have high risk of cardiac dysfunction during pregnancy.Serum myocardial enzyme and echocardiography should be closely monitored in the second trimester.
