摘要
目的探讨MRI在小儿病毒性脑膜炎诊断中的应用及其可靠性。方法选取2018年9月至2019年5月该院收治的疑似病毒性脑膜炎患儿208例,所有患儿均进行CT检查及MRI检查,并以临床诊断结果作为“金标准”。观察比较两种不同检查方式在病毒性脑膜炎中的诊断结果、诊断效能,以及病毒性脑膜炎与非病毒性脑膜炎在MRI诊断分级中的分布差异。结果CT检查共检出病毒性脑膜炎151例(占72.6%);MRI检查共检查出病毒性脑膜炎160例(占76.9%),MRI在病毒性脑膜炎方面的检出率显著高于CT,差异有统计学意义(P<0.05)。CT检查在病毒性脑膜炎中的诊断准确率为86.5%,灵敏度为89.2%,特异度为78.4%;MRI检查在病毒性脑膜炎中的诊断准确率为91.8%,灵敏度为95.5%,特异度为80.4%。MRI诊断效能显著高于CT,差异有统计学意义(P<0.05)。病毒性脑膜炎的MRI诊断分级显著低于非病毒性脑膜炎,差异有统计学意义(P<0.05)。结论对病毒性脑膜炎患儿的诊断过程中,应用MRI诊断的效能更加显著。
Objective To investigate the application and reliability of MRI in the diagnosis of viral meningitis in children.Methods Totally 208 children with suspected viral meningitis admitted to our hospital from September 2018 to May 2019 were selected,all of whom underwent CT and MRI examination.The clinical diagnosis results were used as gold standard.The diagnostic results and efficacy of two examination methods in viral meningitis were observed and compared.Differences in the distribution of viral meningitis and non-viral meningitis in MRI diagnostic classification were analyzed.Results Totally 151 cases(72.6%)of viral meningitis were detected by CT examination while 160 cases(76.9%)of viral meningitis were detected by MRI.The detection rate of MRI in viral meningitis was significantly higher than that of CT(P<0.05).The diagnostic accuracy,sensitivity and specificity of CT in viral meningitis were 86.5%,89.2%and 78.4%,respectively.The diagnostic accuracy,sensitivity and specificity of MRI in viral meningitis were 91.8%,95.5%and 80.4%,respectively.The diagnostic efficacy of MRI was significantly higher than that of CT(P<0.05).The MRI diagnostic grade of viral meningitis was significantly lower than that of non-viral meningitis(P<0.05).Conclusion MRI is more effective in the clinical diagnosis of viral meningitis in children.
