核磁共振成像与脑脊液分析在中枢神经系统感染鉴别诊断中的价值 被引量:24

Application of magnetic resonance imaging and cerebrospinal fluid analysis in identification of central nervous system infections

摘要 目的探讨核磁共振成像(MRI)分析和脑脊液(CSF)中乳酸脱氢酶(LDH)、乳酸(LA)和肌酸激酶(CK)在病毒性脑膜炎等中枢神经系统感染性疾病中的鉴别诊断应用,为临床中枢神经系统感染性疾病的诊断和治疗提供参考依据。方法选取医院2012年11月-2013年10月收治的95例中枢神经系统感染患者为研究组,同期选取30例头疼患者作为对照组,比较两组患者各项指标差异,数据采用SPSS 19.0软件进行统计分析。结果 MRI检查研究组阳性率为98.95%,明显高于对照组的13.33%(P<0.01),研究组早期MRI阳性率91.58%也明显高于后期24.21%(P<0.01);脑脊液分析中按照化脓性脑膜炎、病毒性脑膜炎和结核性脑膜炎进行疾病分组,其中结核性脑膜炎脑脊液LDH浓度最高;化脓性脑膜炎脑脊液CK浓度最高,3组试验结果差异均有统计学意义(P<0.01)。结论 MRI检查和脑脊液分析对于中枢神经系统感染性疾病的鉴别诊断和治疗具有一定的临床价值,两种方法联合能有效鉴别疾病的感染类型,为临床治疗提供参考。 OBJECTIVE To explore the application of magnetic resonance imaging (MRI) and analysis of lactate de‐hydrogenate (LDH) ,lactic acid (LA) and creative kinas (CK) in the cerebrospinal fluid (CSF) in identification and diagnosis of viral meningitis and other central nervous system infections ,so as to provide reference for the clinical diagnosis and treatment of infectious diseases of central nervous system .METHODS A total of 95 patients with central nervous system infections who were treated in the hospitals from Nov 2012 to Oct 2013 were assigned as the study group ,meanwhile ,30 headache patients were chosen as the control group .The indicators were ob‐served and compared between the two groups of patients ,and the statistical analysis of data was performed with the use of SPSS 19 .0 software .RESULTS The MRI examination showed the positive rate was 98 .95% in the study group ,significantly higher than 13 .33% in the control group (P〈0 .01) .The MRI positive rate in the early stage (91 .58% ) was significantly higher than in the late stage in the study group (24 .21% ) (P〈0 .01) .In the cere‐brospinal fluid analysis ,patients were grouped by purulent meningitis ,viral meningitis and tuberculosis meningi‐tis .The highest concentrations of LDH and CK were seen in cerebrospinal fluids of patients with tuberculosis meningitis and purulent meningitis respectively .The three groups had significant differences in the test results (P〈0 .01) .CONCLUSION The MRI examination and the cerebrospinal fluid analysis had certain clinical value for differential diagnosis and treatment of infectious diseases of central nervous system .The two methods can be com‐bined effectively to identify the infection types of the diseases ,so as to provide reference for clinical treatment .
