摘要
目的总结急性肠脂垂炎患者的临床及CT影像学特征。方法回顾性分析10例急性肠脂垂炎患者的临床及CT检查结果。结果10例患者临床表现为左下腹痛8例,下腹部中央区痛1例,右下腹痛1例。CT平扫示病灶位于乙状结肠近端前方8例,乙状结肠远端1例,升结肠旁1例;病灶大小9mm×12mm^13mm×26mm;表现为紧邻肠壁的卵圆形脂肪密度灶,周围伴软组织密度环,周围见少许渗出影9例,邻近腹膜增厚9例,局部肠壁均未见明显增厚。10例均给予镇痛、敏感抗生素治疗,7d后临床症状减轻或消失,复查CT示病灶消退。结论急性肠脂垂炎病灶多位于乙状结肠近端前方,临床以左下腹痛多见,CT平扫可见紧邻肠壁的卵圆形脂肪密度灶、周围伴软组织密度环,并可见渗出影、腹膜有增厚,一般预后较好。
Objective To explore the clinical manifestations and CT features of acute epiploic appendagitis.Methods The clinical data and CT findings of 10patients with acute epiploic appendagitis were retrospectively analyzed.Results Ten patients were clinically demonstrated by left lower abdominal pain in 8patients,central area of lower abdominal pain in 1,and right lower abdominal pain in 1.CT scan revealed that the lesions were located in the front of the proximal sigmoid in 8patients,in the distal of the sigmoid colon in 1,and in the ascending colon in 1,with the size of the lesion ranging from 9mm×12mm to 13mm×26mm.CT showed oval fat-density foci adjacent to the intestinal wall,surrounded by a ring of soft tissue density.Exudation was seen around the foci in 9and the adjacent peritoneum was thickened in 9,with no obvious thickening of the local intestinal wall.All patients received analgesic and sensitive antibiotic treatment for 7days,and the clinical symptoms were alleviated or disappeared.CT reexamination showed regression of foci.Conclusion Acute epiploic appendagitis lesions are more commonly found in the proximal front of the sigmoid colon,clinically presenting left lower abdominal pain.CT scan shows oval fat density foci adjacent to the intestinal wall,with surrounding soft tissue density rings,exudations and peritoneum thickening.Symptomatic treatment usually achieves a good prognosis.
